⏳Processing time:
• Up to 2 business days during the week
• Up to 3 business days for requests made on Fridays or over the weekend

💲Cost:
$30.00 per request
Please note: If you hold a valid Concession Card (Pension or Health Care), please call the clinic to arrange a bulk billed telephone consultation instead

🚨 Need it urgently?
If your medication is needed urgently, please book an appointment with your preferred GP

📋 What if my request is not approved?
All requests are reviewed by a Doctor. If needed, you’ll be advised to book an appointment to ensure you receive safe and appropriate care.

Are you living with a health condition that lasts 6 months or more? Your GP may be able to create a Chronic Condition Care Plan to help you stay on track and better manage your health.

Common conditions include:
• Diabetes
• Asthma or COPD
• Heart disease
• Arthritis
• Chronic pain
• Some Mental health conditions
• And many others

What are the benefits of a Care Plan?
🌸 Personalised health goals
🌸 Better coordination of your care
🌸 Medicare rebates for up to 5 allied health visits per year (such as physiotherapy, podiatry, dieticians, exercise physiology and more)

💡 Please note: Allied health providers set their own privately billed fees. You will PAY THEIR FEE ON THE DAY and receive a Medicare rebate back.

💚 At Health by Nurture:
✔ Care Plans are bulk billed for patients with a valid Medicare card with all GPs
✔ Up to 4 bulk billed Care Plan appointments each year with your GP
(1 x initial long appointment + 3 x review appointments per year, 3 months apart)

What happens at the appointment?
Your first visit includes time with our practice nurse, followed by a long consultation with your GP to create your personalised plan.

This Mother’s Day, we’ve organised a donation drive to provide meaningful and practical gifts to local women in need.

We are collecting new, unused, warm and comfy essentials, including:
• Pyjamas
• Socks
• Leggings
• Track pants
• Beanies
• Alternatively, a small Mother’s Day gift is also very much appreciated

When: Now until Friday, 8th May 2026

Why: To help local women in need
