🦶CASE STUDY: Bilateral Forefoot Pain🦶
After a holiday with excessive walking in poor footwear and increased barefoot time at home, the patient developed persistent central forefoot pain for 4 weeks, described as “like walking on a stone,” with night time throbbing.

KEY SUBJECTIVE FINDINGS:
🔹 Pain locations:
Left: 2nd proximal phalanx & 2nd/3rd intermetatarsal space
Right: 3rd proximal phalanx & 3rd/4th intermetatarsal space

🔸 Structural & pressure-related findings:
Obvious V sign: L 2nd/3rd & R 3rd/4th
High Zebris plantar pressure at the central forefoot
Forefoot corns present: More prominent on L 2nd & R 3rd MTPJ

🔹 Functional impairments:
Reduced lesser digit flexion strength (poor paper grip test)
Soreness at the top of a calf raise

THE TREATMENT PLAN:
🔸 Footwear modification: Stiff rocker-soled shoes to reduce excessive forefoot loading.
🔹 Taping support: Ribbon taping to offload plantar plates and improve function.
🔸 Targeted strengthening: Exercises to improve lesser digit flexor strength and stability.
🔹 Referral for opinion for corticosteroid injection: To manage persistent pain and inflammation.
🔸 Orthotic intervention: Custom orthoses featuring a metatarsal bar and forefoot valgus padding to redistribute pressure across the entire foot.
🔹Callus and corn debridement: To reduce pressure and discomfort in high pressure areas.

TAKEAWAY:
Forefoot pain isn’t just about rest! It requires load management, a tailored treatment plan, and strengthening to address both symptoms and underlying biomechanical factors.
