📢 NDIS Update – New Planning Changes (Coming Mid-2026)
The Australian Government has announced a new way of NDIS planning called new framework planning.

🔹 No action is needed right now
🔹 Current NDIS plans will stay in place
🔹 Changes will roll out gradually from mid-2026
🔹 Children under 18 are not included initially

The new approach will include a support needs assessment — a guided conversation with a trained assessor to better understand individual support needs. The aim is to make plans fairer, clearer, and easier to use.

Under the Support at Home program, care management isn’t a one-off task — it’s ongoing support to make sure your services continue to match your needs as they change.

At Care to Care, care management means:
• Regular check-ins to see how things are going
• Reviewing services as your needs or circumstances change
• Coordinating supports and third-party services
• Clear communication and help navigating the system
• A local team you can contact when questions come up

Care management fees are set by the government and form part of how providers coordinate, monitor, and support your services under Support at Home.

Welcoming the New Year with gratitude, joy, and beautiful moments 💛

As we step into the New Year, we’re reflecting on some truly special moments our Support Workers shared with us in December.

One of our amazing Support Workers took a client to the Ashes cricket, where they had an absolute blast - laughter, excitement, and memories made. Another spent Christmas Day with a client, ensuring he wasn’t alone and felt cared for on a day that can be difficult for many.

Seeing the smiles on our clients’ faces reminds us why we do what we do.

A heartfelt thank you to Nicole and Sharon for your kindness, compassion, and dedication. These moments mean more than words can say.

We were absolutely thrilled to receive these beautiful photos from our amazing Support Worker Sharon, who recently attended an art show with one of our talented clients, Ruby, and her family.

Out of 650 entries and 59 finalists, Ruby’s incredible artwork shone brightly, winning the Emerging Artist Prize on opening night, and then taking home People’s Choice on closing night! 🌟

What an amazing achievement, Ruby your creativity and passion truly inspire us all. We’re so proud of you and the beautiful connection you share with Sharon. It’s moments like these that make our work so meaningful. Thank you for sharing with us all. 💕
