Mechanical Knee Pain: Often an Alignment and Function Problem

One question we hear often is:
"Why on earth would I see a chiropractor for knee pain? Don't they only crack spines?"
The short answer: No, we don't only work on spines — and at Cannon Hill Family Chiropractic, we don't "crack" anything at all. What Does Pain Really Mean? Insights from Pain Science and How We Can Help at Cannon Hill Chiropractic

Pain can be confusing, frustrating, and scary—especially when it's persistent low back pain that seems to linger without an obvious cause. Many people assume pain always equals damage: a "slipped disc," a torn muscle, or something "broken." But modern pain science tells a different, more empowering story.
One of the world's leading pain researchers, Professor Lorimer Moseley, describes pain as a sophisticated protective mechanism generated by the brain. In his work (including the popular "Tame the Beast" explanation and the book Explain Pain), Moseley explains that pain isn't a simple signal from damaged tissues—it's the brain's way of saying, "Hey, protect this part of your body!" It's an alarm system designed to change your behaviour, promote healing, and avoid potential harm.
Pain is real—always. But it doesn't always mean structural damage is present or ongoing. You can have significant tissue changes (like wear and tear on a disc) with little or no pain, and you can experience intense pain with minimal or no visible damage. The brain weighs up a huge mix of inputs: actual tissue signals (nociception), past experiences, beliefs about the injury, stress, sleep, emotions, and even social context. If the brain decides the threat is high, it dials up pain to motivate protection—even if the tissues are safe.
This is why understanding pain science can dramatically help. Research shows that learning accurate information about pain (often called "pain education") can reduce pain intensity, fear of movement, and disability—especially in chronic cases. When people realize pain is protective rather than destructive, they often feel less threatened, move more confidently, and recover faster.

Here are some key concepts from pain science to remember:
*Pain is real — It's a genuine experience, never "all in your head."
*Pain does not equal damage — The link between tissue state and pain weakens over time, particularly in persistent pain.
*Pain is for protection — It's your brain's way of keeping you safe, often acting as an early warning before damage occurs.
*Pain can be influenced — Many factors turn the volume up or down: thoughts, beliefs, activity levels, stress, and more.

There are many mechanisms or influencers of pain — Beyond tissues, things like central sensitization (where the nervous system becomes over-sensitive) can amplify pain even when healing has occurred.
A helpful acronym I like to share with patients is PAIN:

If you're experiencing pain—especially if it's lasted more than 3 months (chronic pain)—don't ignore it. Pay attention and get it checked. Pain is your brain's smart way of prompting behaviour change to reduce risk. Our bodies are adaptable and intelligent; listening to pain early can prevent it from becoming more complex.

What is Causing my Low Back Pain?

Is it a disc? I think it is muscular! Are these the only two options?
I find it interesting that people who have back pain will go to these two options. Having looked after many people with back pain every day in clinic I know that these are not the only two generators of pain.
When you experience low back pain it is not fun. Whether it is acute or chronic, low grade pain or intense, the pain from your low back can take your energy, take your focus and definitely make you frustrated; Not to mention it can make you a lot less productive. You tell your friends, and they all have a story of their own back pain or know someone close to them. Their stories do not always help, and when it comes to what might be wrong, so many people will question whether it is a disc problem or sciatica. These are valid questions, and this reinforces the point to have it looked at by someone who knows what they are talking about. There are many reasons to do this, such as making sure it is not something nasty, getting strategies to help your pain, and ensuring you are not doing things that may create damage or slow your progress.
Back to the causes…
Discs sit between vertebrae and indeed can be a cause of pain. It is important to note that when it comes to back pain, we do not like pain that travels down the leg. There are other structures that can also cause your pain, such as the facet joints that connect your vertebrae or your SIJ (Sacro Iliac Joints) that connect your pelvis posteriorly (in the back). It could be coming from your hip or even your p***s. It could be from a muscle like the piriformis, gluteal muscles, or the QL (Quadratus Lumborum). Is it a fatigue of muscles that are not fit, and they begin to ache or recruit other muscles to do the job, leading to inefficiencies? Is it inflammation? Is it a result of one of these things or many? Is it some other structure or problem you have never heard of?
This is why you want it checked out. Many times, low back pain is not specific. It is not related to just one joint or muscle or tissue. It is often a biomechanical problem where joints are not moving properly, muscles are not activating to move joints and protect them (remember that muscles don't just move joints they protect them too). This change in biomechanics loads joints and muscles inefficiently, leading to poor transfer of force through joints and inappropriate loading that can trigger pain.
If you stick with us we will explain a little more about pain in an upcoming post, however it is important to understand that pain is a signal that should not be ignored, it is not always a result of damage but it can be. It can also be a signal of potential damage. So, as mentioned, it is best to get things looked at and treated if necessary so things do not lead to damage or further damage if it has already occurred.
