One of the most common things we hear is, “my bite feels off.”
But not every case is driven by occlusion.
In many TMD presentations, that sensation can be the result of joint compression and altered proprioception, where the patient’s perception of how their teeth meet changes because the joint is under load.
This is where cases can easily go off track.
If the driver is the joint rather than the bite itself, repeated occlusal adjustments may not only miss the cause, they can sometimes make symptoms worse.
Understanding whether the issue is joint, muscle, or true occlusal change is often the key to better outcomes.
A helpful reminder that in TMD, what the patient feels is real, but the source is not always where we first look.
Is this something you’re seeing more often in practice?
