Bad fracture dislocation of the ankle. Normally I would place 2x syndesmosis buttons, but because the deltoid was repaired with anchors, I managed to get a really good reduction of the joint space, so managed to get away with 1x button just to make sure the syndesmosis heals.

Don't underestimate the power of the deltoid repair 🦾

Significant ankle instability repaired through a minimally invasive ArthroBrostrom approach.

2x anchors are inserted into the fibula to repair the ATFL and CDL ligaments, passed through the distal incision grabbing the extensor retinaculum and tied back on providing stability.

Less pain, quicker recovery and reduced risk of infection from wounds.

First use of the Paragon28 Reflex syndesmosis stabilisation system in Australia!
Thank you Paragon28 for allowing me the ability to use the Reflex system for this post traumatic ankle arthritis patient.
The patient had a previous syndesmosis injury not stabilised hence leading to arthritis. I knew I wanted to insert a suture button but it would compromise the insertion of the Inbone stem.

Hence the Reflex was inserted prior to the Inbone stem going in. Deltoid ligament also repaired.

From having a significantly plantarflexed foot to finally being able to put the foot down on the ground.

Still stiff, however to come from 0 deg motion in ankle to having motion is a game changer for mobility.

Typical Weber B fracture ankle with talar shift/increased medial clear space. More and more I've been opening and repairing the deltoid medially.

I know the evidence is still out there on it's outcomes, but it certainly provides significant stability to the ankle, hence the use of only 1x suture button instead of my usual 2. As per the literature the medial clear space is better reduced, so long term, the clinical outcomes should be better right? 🤷

Some of the largest fragments of bone I've taken out from the posterior aspect of the ankle!
An os trigonum can be a normal accessory bone located around the back of the ankle, however in this case, associated with arthritis of the ankle can enlarge to quite significant sizes causing pain and impingement.

Elderly 78 year old lady, diabetic, fracture blisters and an undisplaced Weber C #.

The best indication for a minimally invasive fibula nail. Small cuts, better healing, immediate weight bearing and blisters have dried up well with use of silver dressings at 2wks post op.

Significant flat foot correction with triple arthrodesis. Stay tuned for post op clinical pics.
Admittedly with such a significant deformity it is often hard to recreate a normal looking foot can I find the heel still remains and slight valgus but to the patient it will be a significant enough correction.
