Typical Weber B fracture ankle with talar shift/increased medial clear space. More and more I've been opening and repairing the deltoid medially.

I know the evidence is still out there on it's outcomes, but it certainly provides significant stability to the ankle, hence the use of only 1x suture button instead of my usual 2. As per the literature the medial clear space is better reduced, so long term, the clinical outcomes should be better right? 🤷

Some of the largest fragments of bone I've taken out from the posterior aspect of the ankle!
An os trigonum can be a normal accessory bone located around the back of the ankle, however in this case, associated with arthritis of the ankle can enlarge to quite significant sizes causing pain and impingement.

Elderly 78 year old lady, diabetic, fracture blisters and an undisplaced Weber C #.

The best indication for a minimally invasive fibula nail. Small cuts, better healing, immediate weight bearing and blisters have dried up well with use of silver dressings at 2wks post op.

Admittedly with such a significant deformity it is often hard to recreate a normal looking foot can I find the heel still remains and slight valgus but to the patient it will be a significant enough correction.

MIS bunion correction. Again noting benefits of 3D correction ensuring the rotation of the toe is corrected at time of surgery. In most cases, if this is corrected, there is minimal to no need to "shave" the actual bunion away.

Rare case of a chronic FHL tendon rupture to the great toe. 1st pic showing a large gap approximately 6cm after retraction. This required a plantaris tendon allograft to bridge the gap.

6 months post op and able to begin a single heel raise, with a well healed wound. Still minimal flexion at the IPJ, but this is unfortunately what literature reports as well. Toe is at least sitting in better position with less aching.

Another seemingly innocuous Weber B fracture that ended up requiring a deltoid repair as well due to increased valgus tilt and medial clear space widening.

Another flat foot reconstruction, the heel wasn't too valgus, but noted abduction and talonavicular uncoverage, so an Evans was performed instead along with a Cotton osteotomy and TAL. The tib post tendon was noted torn only, so this was repaired and not transferred.

Trimalleolar fracture dislocation of the ankle. Open fixation of all 3 components, in particular use of the postero-medial approach rather than a standard posterolateral, which can decrease incidence of peroneal artery injury causing stiffness to the ankle via necrosis of the FHL muscle.

Weight bearing after 2 weeks in a Cam boot, as these are prone to developing stiffness post op.
