Varus ankle arthritis from chronic instability, the high arch certainly doesn't help.

Total ankle replacement performed with Inbone stem, lateral ligament stabilization and calcaneal valgizing osteotomy to try our best offloading the implant as best as possible.

Not all bunions can be managed through percutaneous or minimally invasive techniques. This patient in particular and arthritis of the 2nd TMTJ and instability of the first Ray. Therefore a Lapidus fusion of the 1st and 2nd TMTJ combined with a MIS Akin can still offer good correction of the bunion and treat the arthritis simultaneously.

Complete rupture of the tibialis anterior tendon 4 months ago. The telltale lump in 1st picture is retraction of the tendon under the extensor retinaculum. The second picture showed a defect with retraction of almost 7cm, which we will unable to bridge by a end to end repair so an allograft tibialis anterior tendon was used to bridge the defect.

Important to have good repair of the extensor retinaculum back over the tendon to prevent bow stringing. Good closure of the wound at the end.

Another ankle replacement this time in significant valgus. Good correction post op with an Inbone stem, you can see where the calcaneus was impinging against the fibula in the post op AP X-ray.

Included a deltoid reconstruction and shifted the calcaneus, better restoration of the arch on the post op lateral x-ray as well.

This time a deltoid using a LARS ligament. Two anchors are placed for repair of the deltoid initially then the LARS is placed in the medial malleolus to create a two-tailed graft.

2 tunnels placed respectively in the talus and sustentaculum to recreate the superficial and deep deltoid ligaments. The remaining anchors are then passed through the deltoid ligament and the graft tightened with the ankle in neutral.

77yo for a total ankle replacement. Slight valgus with pre-existing flat foot. Chose an Inbone as these are rock solid and offload the joint better IMO. Shifted the calcaneus into varus for better Achilles alignment. Didn't feel I needed to repair/augment the deltoid after trial implants.

The widening between the base of the metatarsals consistent with a Lisfranc injury. This ligament is important for the stability of the midfoot and the arch. Gross instability can lead to long-term arthritis.

The next X-ray shows the importance of comparison away bearing views.

Is patient had significant hypermobility and despite a previous lateral ligament stabilization with suture brace they progressively loosened and became unstable again.

I had already demonstrated the LARS reconstruction technique but just wanted to show a better picture of the location of the tunnels used on x-ray. The lateral demonstrates position of the CFL tunnel and the pre-existing anchor in talus used for the suture brace, so had to go slightly more anterior for the new ATFL attachment. The AP x-ray shows the fibula tunnel.

Dr Thayaparan is a South Australian trained surgeon looking to sub-specialize in foot and ankle surgery and has just completed an arthroplasty Fellowship in hips/knees.

He will be with us for 6 months learning the dark arts of complex/MIS foot and ankle, he had a baptism of fire with a complex total ankle replacement on our 1st list together.

Achilles avulsion from the attachment to the calcaneus, rare but devastating injury. The principal remains that the tendon needs reattachment to bone, however this is not always that easy, and so as reinforcement, a LARS graft was utilized as an only patch to encourage healing to bone, while the tendon was repaired using anchors through a standard double row speed bridge type repair.

16 degrees of varus to the ankle corrected with a total ankle replacement, including a lateral ligament stabilization and MIS calcaneal valgising osteotomy.
