Our head office is based in Clermont in remote Central QLD.

Stream Services provides NDIS supports to Australians in rural and regional areas, offering a range of high-quality services including Supported Independent Living (SIL), Lifestyle Support, Support Coordination and Allied Health Services. Stream Services is a newly established NDIS teleservice which is focused on supporting people in regional and remote communities and people who are otherwise physically isolated who may due to their location find it difficult to access the services that they require. With care we tailor our offering to the needs of our clients – both actual NDIS participants, the advocates and the coordinators that support them. We are a registered NDIS Service Provider, who has carefully selected a qualified and friendly allied health care team that consists of a Psychologist, Exercise Physiologist, Occupational Therapist, Dietitian, Coordinator, Counsellor, Allied Health Assistants, counsellors and Lifestyle Support Workers. Our team has been chosen from all over Australia as we wanted the best practitioners to support our all of our clients. Our team is available to chat with you about your individual needs, the barriers that you may have with accessing providers due to your location and how we may be able to help you to reach your goals.

Our Supported Independent Living (SIL) house in Clermont reflects what Participants value most: safety, respect and connection.

Our Support Workers provide consistent, person-centred help, but the environment itself belongs to the Participants.

They choose how their spaces look and feel, build routines that suit their day, and create a home life that reflects who they are.

It’s a place where Participants can grow their independence while staying connected to their community, family and the things that make life meaningful.

Every person’s day looks different.

That’s why our support is shaped around what feels familiar, comfortable and meaningful to each Participant.
We start by getting to know the person and their family – who they are, what they enjoy, what helps the day run smoothly, what routines matter, what brings confidence and what feels safe.

From there, we build support that fits real life. That can look like hands-on assistance at home, guidance with cooking and healthy eating, gentle strategies that make everyday tasks easier, steady routines, or opportunities to stay involved in the community.

When support reflects the person first, life becomes easier to manage, more connected, and filled with moments that feel genuinely their own.

It was a chance to slow down, share a meal, enjoy each other’s company and celebrate the moments that have shaped this year.

So many people have reached meaningful milestones, grown in confidence and strengthened their connections across their communities.

We’re grateful for every person who walked alongside us this year and for the chance to celebrate together.

We know the importance of Participants staying close, not just to family, but to all of these familiar things.

Alec McConnell founded Stream Services, originally known as Clermont Country Care, in 2019 after hearing a plea for help from a rural GP who couldn’t find disability supports for her patients.

What began as a local response to a community need has grown into a regional service helping people in country and outback Queensland live meaningful, independent lives close to home.

With a background in community engagement and government, Alec has dedicated his career to improving access to quality care and support in rural and remote areas. His vision is simple: to make sure people don’t have to leave their communities to receive the services they deserve.

Under Alec’s leadership, Stream Services continues to expand its Supported Independent Living, Allied Health, and community support programs across Queensland - delivering person-centred care that values respect, choice, and connection.
