As per, I'm sitting here in the middle of a debate thinking, "you're both right."

Pop psychology (PP) can be simplistic, ableist, racist, a little too optimistic.

But the opposite is having a moment - completely undermining ANYTHING that (or comp medicine) has to say.
PP is well, popular, because there was a NEED. People can't attain a standard of joy, success, PERFECTION that is plastered across our screens and it hurts and they want to know WHY & PP is offering a vocabulary for that hurt. So much was behind closed doors and now that it's out in the open we're wondering why we can't keep up with the Jones instead of just keeping up.

BUT PERSONAL RESPONSIBILITY cries the PP haters!

Well actually YES. It's all well and good for me to say, “I struggle to do xyz because things weren't always easy / modelled for me, I wasn't taught about investing, my emotional needs weren't met". But I am adult who has responsibilities:
🌼to those around me (family, employers, friends, the community at large and MY OWN HEALTH.

And we do need to appreciate that we can't tell all these people that we find it hard to keep up because as a child xyz happened, or that we have been a victim of body dysmorphia or every other micro trauma that has bloody occurred.

BUT

Ugh. The juggle.

As a health professional, I actually like to jump right into the middle here, because tbh I see a need - a gap in the market if you will. It's why I'm a bit more gentle on "THE RULES" or a 'my way or the hwy' approach.

Because it's me as well.

Most of my patients identify as adult women (or teenagers). And you know what - most of them have seen or been told what they must do "to be healthy" & for most of them, it is hard or unattainable & I refuse to GUILT a patient who wants to have a baby or fix an ailment.

And I will scream about James Clear's Atomic Habits forever, but the reality is BOTH POP PSYCHOLOGY AND THE ARGUMENT FOR PERSONAL RESPONSIBILITY ARE CORRECT.

Sometimes there's a timeline and we may gently need to hit the accelerator a bit to get where we're going. Sometimes having someone to answer to in the beginning of a habit change HELPS our accountability.

Very informative post for those interested in reading about out of pocket costs for Medicare based healthcare. Particularly interesting if you have a family / multiple people on one Medicare card.

Of course a lot of people are interested in bulk billing of GP's or lack thereof. This explains a bit of that topic (it's a finding conversation), but mostly how you can best have the system work for you. :)

Many of you would be aware that many GPs have moved away from the bulk-billing model due to the rising costs of running a small business. With more GPs implementing a mixed-billing or private billing model to remain sustainable, for the first time many patients are now experiencing out of pocket costs when they see a doctor.

I have previously spoken about how the freeze on Medicare rebates, over successive governments, has resulted in a ballooning deficit when it comes to the costs of providing medical services. While many patients may be dismayed at having to pay to see a doctor they previously saw ‘for free’, tonight I wanted to discuss a Medicare initiative that can help lessen the cost of medical care for those who have large families, are concession card holders, receive Family Tax Benefit A, or who have frequent medical appointments.

Many patients are unaware about the existence of the Medicare Safety Net. Many more may have heard of it, but are unaware that they need to register for it with Services Australia.

So, what is it?

The Medicare Safety Net is available to everyone who is registered with Medicare. For simplicity’s sake, I will discuss the Extended Medicare Safety Net, because this is the program that can have a big impact on saving money when you see doctors for out of hospital services.

So, how does it work?

Medicare will normally pay you a rebate for the medical services that you have accessed through them. Bulk billing doctors would ordinarily accept that rebate alone as sufficient payment for their services without charging you anything out of pocket. But with bulk billing on the decline, your doctor is charging you a fee for their services because the rebate in insufficient to remain viable. This results in you being out of pocket a certain amount. In GP land, this out of pocket payment will vary depending on the time spent with the doctor, and the indication for the appointment.

However most people don’t recognise that when you have paid a certain amount in out of pocket payments for medical services, Medicare’s Extended Safety Net kicks in. This means that Medicare will now pay 80% of the out of pocket costs, slashing the amount you are paying.

To illustrate, Jane goes to her GP every few weeks for management of several chronic health concerns. Each appointment costs her $85 (the recommended AMA rate) but the Medicare rebate only covers $39.7 of this. She is hence $45.3 out of pocket for her consultations.

However, once Jane has reached the Extended Medicare Safety Net threshold, Medicare will cover 80% of her out of pocket costs for seeing her doctor. She is hence $9.06 out of pocket for her consultations for the remainder of the calendar year.

So what is the threshold?

If you are an individual with no concession card then the threshold is reached when you have paid a total of $2249.80 in out of pocket services.

However, if you are a family with no concession card, then the threshold is reached when the entire family have paid a total of $2249.80 in out of pocket services.

But what people don’t realise is that having your children and spouse listed on your Medicare Card doesn’t automatically register you as a family for safety net purposes 🤯 You MUST fill out a form available on the Medicare Express app or the Services Australia website, explicitly stating that your spouse and dependents are one family unit, otherwise Medicare will consider you as seperate independents with a Safety Net threshold of $2249.80 each, instead of $2249.80 cumulatively.

So let’s get back to the example of Jane.

Jane is seeing her doctor every few weeks for her chronic medical needs, paying $45.3 out of pocket for her appointments. Her 18 year old daughter, Melanie, has been seeing a gastroenterologist monthly for her Crohn’s disease, paying $200 out of pocket for each appointment. Her 21 year old son, Paul, has been seeing a sports physician every fortnight for an acute and serious knee injury sustained playing soccer, and he has been paying $200 out of pocket for his consultations.

But because Jane registered her two dependents Melanie and Paul as her ‘family’ with Medicare, once they have collectively paid $2249.8 in out of pocket costs, the extended safety net kicks in. Jane now is $9.06 out of pocket for her GP visits, Melanie is $40 out of pocket for her gastroenterologist appointments, and Paul is $40 out of pocket for his sports physician appointments.

But there is more.

Because if the family is a concessional family, where two of the people in the registered Medicare Safety Net family have an eligible concession card, OR the families are eligible for Family Tax Benefit Part A, the safety net threshold is much, much lower: $717.90!

Continuing Jane’s example: Jane’s and Melanie’s chronic medical conditions mean that they are both on Disability Support Pensions. As a result of this, the family’s Safety Net Threshold is now $717.90. Once the family have collectively paid this amount in out of pocket fees, Medicare will cover 80% of their out of pocket medical expenses.

So, what services counts towards the safety net?

Out of hospital services with your GP and other medical specialists will count, as will radiology and pathology services.

But what many people don’t realise is that services like psychology also count! Anyone with a mental health care plan knows how cost prohibitive regular psychology sessions can be, but the Medicare Safety Net will slash the out of pocket costs associated with it.

But remember: anyone who is part of your family MUST register with Medicare as a family for Safety Net purposes. It is not automatic and it doesn’t matter if you are all listed on the same card. Go to the Medicare Express app or Services Australia website to do so.

I hope that the above information provides some relief to families that are experiencing hardship during these trying times.

This is the chocolate my best mate bought me when I had a termination a few years ago.

*For those who know me, it wasn't with V just to be clear.*

It wasn't an easy decision - in fact it was awful. Had I continued with the pregnancy I can assure you my mental health would be devastated as the person was not good to me (emotional abuse is abuse too sweet angels ❤).

I now know that if a day should come when I find myself a mum, I will be financially and emotionally more stable and capable of giving that person what they deserve.

AUSTRALIA IS NOT IMMUNE
12 years ago a friend I met on a road trip in the NT fell pregnant despite having an IUD. English was not her first language. When she went to a doctor (hospital?) to ask about getting a termination she was told she couldn't ask for that and he asked her to leave if she was going to continue to request one.

The doctor's wife then came in - she was a nurse. They offered for my friend and her partner to go and live with them to see out the pregnancy and give birth as a 4WD is no place to experience pregnancy - they were road tripping.

She kindly said no thank you (kinda like the Homer gif where he disappears into the hedge) and drove to SA to enquire - it would have been cheaper to fly home, terminate then come back to Aus. THEN they drove to Melbourne where it was affordable.

They ultimately broke up at the end of the trip. She is now married with 2 children. I don't know the full story of how far along she was or what the laws were but I ended up calling my mum to ask what she knew about abortion because we were so confused.

I know everyone's sharing and it's hard to read and it's awful and I'm sorry to those who will lose this life saving and life enriching medical intervention.

I'll keep my ears to the ground for positive places to funnel my outrage and despair - not just via Instagram.

Take a break from the flood of outrage and trauma sharing if you need to and come back when you feel strong.

These are dark days. And I feel the stress of coming back on Instagram and commenting on world events that are awful / heartbreaking.

But I also wanted to inject some humour. Perhaps that is out of turn. And I do apologise if it's not ok. I hope you can feel my fury and devastation.

Beyonce seemed relevant, given her most recent drop culturally and due to the title and lyrics - it felt a good fit. "You won't break my soul."

But moments before posting, I found myself googling "Is beyonce pro-life?" My 28 seconds of research neither confirmed nor denied, but even that I questioned it made me feel uneasy and that the content of the memes was not a good fit.

But I went with it - either because I thought it added to the commentary or out of laziness - maybe both. In using her persona for a meme she might disagree with, I think it adds to the conversation of whether you can separate the artist from their art. My heart still sinks when I think of my favourite film The Pianist (not The Piano).

But do look at the last one - and think of me when you do. 💩

Anyway. What a weekend.

I'm a sucker for flattery and today one of my patients said she misses my posts and asked how my week off Instagram was.

Guys. It's been so nice 😍🧖‍♀️. Allow me to be SO boring for a second, but I feel so UNENCUMBERED. 10/10 Would recommend. Uninstall the app so you can't accidentally go in.

So basically, I'm here to say, I'm probably going to take the better part of the next week off as well, (except for meme Saturday which I'm gagging for) 💁‍♀️

But also, it feels nice to say that 9/20 of my patients today were pregnant. Literally from 4 week to 40 weeks and it was so very special and reminds you just how different it is for everyone.

I hope your stress is manageable and your chips have been delicious this week.

I'll set up a little out of office for my DM's, but this post makes sense too I think.

Love you, miss you, but I have a little pile of work projects (that you'll enjoy) that I'm just not getting anywhere near, because I'm busyyyyyy (praise jesus) and spend a lot of time here 💁‍♀️

There are different paces to feeling better / getting better.

Sometimes it's about maintenance.
🌱 You may not want to change your lifestyle or you CAN'T change it (parenthood, work etc etc), but you require MAINTENANCE to keep you feeling capable or on top of the world.

Sometimes it's super-speed.
🌱 We move quickly, I give advice and (try to) hold you to it, supportively.
🌱 Your symptoms or diagnosis might be severe and we need to or want to move quickly.
🌱 Sometimes fertility falls into this category.

Sometimes it's on and off.
🌱 Health stuff comes up - you come see me. We work through it. Then I see you when I see you.

Sometimes it's just nice.
🌱 There's nothing in particular going on - you just like acupuncture / cups / me.

I'll always try to consider what habits or lifestyle factors could help you on your way. And I'll give them to you as an option with reasoning, but it's your life and you do you ♡.

Knowing a little is good, advocating for yourself is good, but please don't feel like you need to know EVERYTHING in minute detail about your health.

Broad strokes are good.

I pay my accountant to take care of the financial aspect of my business and she is VERY patient and kind with me (lol).

I... think it's great that there is so much free information available on Instagram, tiktok, blogs etc, but... mate, I'm a registered health professional and even I find it overwhelming!!

You can't know it all. And you pay us to take care of you. Ask questions, be interested, but don't feel like you need to know the detail.

THAT SAID, it's complex, because you want to know that you can trust your health professional or doctor. So, yes, self advocacy is important.

And those with chronic health conditions know all too well, that they NEED to know a lot to ensure they're being taken care of. That's why there's such an abundance of endometriosis forums - because if endo survivors aren't informed, they are at risk of being mis-treated.

In the case of me with my patients, I want to be able to set you up to know when to seek help and also to have some tools at home up your sleeve. Basically I want to set you up with tools that aren't just panadol, cold and flu pills or va**um to soothe the heath anxiety that sets in when you investigate too much 💁‍♀️

In short - please don't get overwhelmed by the abundance of information on this and other platforms. Ignore it sometimes. Stay in your lane. Have a cup of tea. ❤

We can't all be fitfluencers. I for one am not. But this is the advice I give to patients who feel like they're struggling, but want to find time for exercise.

Don't put expectations on yourself that you can't meet.

3 x yoga classes or whatever SOUNDS so doable, sometimes (a lot of the time) it's HARD to fit in.

The reason I suggest starting so small is that if you get to Friday and you've set a goal of 3 classes and have only been to one (or none) you enter the weekend feeling like s**t.

It kinda sets you up for failure.

If you pick ONE class at ONE time, once you do it, you're DONE for the week.

It could be a jog, a walk, a HIIT class, a YouTube yoga class, WHATEVER. But set a time for it.

Don't begin with a membership. Start with single visits.

Let the dopamine hit of achieving something validate your experience.
