What services does DVA Community Nursing provide and how do I access it?
If you’re a veteran and need help at home, DVA Community Nursing provides clinical care in your home. It’s for veterans with a Gold Card, or a White Card if the condition is accepted by DVA.
This can include services like wound care, help with medications, diabetes support, continence care, support after a hospital stay, overnight care or end of life care at home. The support is based on your health needs, not a set package or budget.
To get started, you simply ask your GP to complete a DVA Community Nursing referral form. Once we receive that referral, one of our Registered Nurses will visit you at home, complete an assessment, and organise the care you need.
