We can't state enough the importance of completing the required onboarding documents prior to participation in the RadioVoice program.
This has been highlighted recently after the parent of a new student dismissed these documents as "unneccessary" and refused to complete or return. The forms ensure we can deliver services safely, professionally, and in full alignment with NDIS expectations. They also allow us to support each participant with clarity, accuracy, and respect.
The Client Details Form collects essential information about the participant, including full contact details, NDIS plan information, emergency contacts, disability details, and any required medication or additional support needs. It also includes plan manager information so invoicing can be handled correctly and without delay. Having this information upfront is critical. It ensures we understand the participantâs specific needs, can respond appropriately in any situation, and can tailor our support to create a safe and positive experience.
The Photo/Video Consent Form gives us permission to capture and share photos or videos that demonstrate the participantâs involvement and progress throughout the program. These materials may be used for communication with families, program updates, or showcasing achievements. Consent ensures respect for privacy and choice. Without it, we cannot share or celebrate the participantâs "high five moments" beyond the immediate service environment.
The Sharing Information Consent document authorises RadioVoice to share relevant information with the participantâs nominated support network when necessary. This may include family members, support coordinators, plan managers, and agencies such as the NDIA/NDIS. This consent ensures everyone involved in the participantâs care and support has accurate, timely information. It helps maintain continuity, avoid misunderstandings, and ensures smoother collaboration between all parties.
The Service Agreement outlines the services we will provide, the responsibilities of each party, fees, cancellation terms, and the expectations that guide our working relationship. It protects both the participant and the provider by ensuring clarity and transparency from the beginning. A signed agreement is essential for compliance and ensures that services can commence confidently and professionally.
Unfortunately, without the required onboarding documents being signed, a young man is now unable to participate. Itâs disappointing, as this new student showed real potential and enthusiasm, and this decision means he now misses the opportunity to build confidence, develop skills, and benefit from a program designed to support his growth.
