Vestibular rehabilitation is a specialised form of physiotherapy specifically designed to help with patients with vestibular dysfunction/dizziness. Depending on the patient’s injury and presentation, vestibular physiotherapist will perform various treatment techniques and design a vestibular rehabilitation program.

With BPPV, once it was diagnosed and the specific affected canal was located, your vestibular physiotherapist will be able to perform treatment maneuver specific to the affected canal and type of BPPV, to reposition to displaced otoconia and many patients with BPPV will improve within 1-3 sessions.

With other vestibular conditions, a vestibular rehabilitation program may be prescribed. It could consist of:

Gaze stability exercise – to help train the coordination between your head and eyes

Adaptation exercise – to help your vestibular system and brain to adapt and recalibrate after a vestibular injury

Balance exercise – to help maximise your ability to stay balance, and reduce the risk of falling

You were told that you have some loose crystals in your ear causing you dizziness?

The are many different conditions that can cause dizziness and vertigo. They can generally be classified into 2 categories - peripheral and central. Peripheral vestibular disorders are injuries or dysfunction of peripheral vestibular system such as “loose crystals” in the semi-circular canal. Central vestibular disorders are injuries or dysfunction of the central vestibular system such as a stroke or multiple sclerosis.

One of the most common vestibular disorders is Benign Paroxysmal Positional Vertigo (BPPV). It is the most common cause for vertigo. It is due to the displacement of some calcium carbohydrate crystals (Otoconia) from the macula to the semi-circular canals. Since the semi-circular canals are responsible for sensing the position and angular movement of our head, and the otoconia disturbs the canals and results in vertigo symptom. The most commonly affected canal is the posterior canal. BPPV can be diagnosed with a detailed subjective history and some specific testing technique by your vestibular physiotherapist.

Another common vestibular dysfunction is unilateral vestibular hypofunction resulted from vestibular neuritis. Vestibular neuritis is a condition in which the vestibular nerve is inflamed, usually due to a viral infection. Patients with vestibular neuritis usually have severe dizziness, vertigo and disturbed balance. Symptoms can last from days to weeks. Some patients may continue to have symptoms after weeks, which is usually due to unilateral vestibular hypofunction resulted from the vestibular neuritis.

Your GP said your dizziness is caused by vestibular system dysfunction?

Do you know what does that mean?

Vestibular system is a system within our ears that monitor the position and motion of our heads in space. The vestibular system coordinates our head and eye movement with sensory input from our eyes and proprioception and works with our brain to ensure we have a stable gaze, stable body and balance.
The vestibular system is consisted of multiple components, such as the 3 semi-circular canals, otolith organs and the vestibular nerve and the vestibular nuclei. Injuries or dysfunction to any of the components or to our central nervous system could result in symptoms such as dizziness, vertigo and cause disturbance to our gaze and balance.

Vestibular rehabilitation is very important in managing recovery from various vestibular injuries and dysfunction. There is good evidence indicating that vestibular rehabilitation can help with improving dizziness/vertigo symptoms, gaze stability, balance and physical function.
