✨ “What actually happens in a pelvic floor appointment?”

So many women delay booking because they’re unsure what to expect.

At Shire Women’s Physio, your appointment always starts with a conversation. We discuss your symptoms and concerns, goals, birth history, bladder and bowel habits, training load and lifestyle.

We use pelvic models like this to explain what might be happening anatomically, and demonstrate what is involved in the assessment of your pelvic floor and vaginal walls.

Internal assessments (where we insert 1 or 2 fingers vaginally) are never rushed and are only performed with your consent. They help us assess muscle strength, coordination, tone and support. This allows us to diagnose what is causing your symptoms and develop an effective treatment plan.

You deserve to feel informed, comfortable and empowered, not embarrassed.

The vicious cycle 🔄 Pain with intimacy isn’t “just in your head” - it’s a very real body-brain–muscle response.

An initial pain trigger (like tampon use, a painful speculum exam, period pain, or pain with s*x) can cause the pelvic floor muscles to tighten and spasm.

Over time, the brain remembers this pain and starts to anticipate it. This anticipation leads to guarding and involuntary muscle tension before anything even happens, which then contributes to more pain.

Eventually, an association forms between s*x and pain, and fear-avoidance behaviours can develop, reinforcing the cycle.

• reducing muscle overactivity and spasm
• retraining the nervous system’s pain response
• improving relaxation, coordination, and confidence

FAQ: Does leakage always mean I should do more kegels??

Short answer: No.

Urinary incontinence is not always caused by weakness of the pelvic floor muscles. While reduced strength can contribute, leakage is often related to how the pelvic floor coordinates, relaxes, and responds to load and pressure.

Here’s what the evidence and clinical practice show:

• Some pelvic floor muscles are low tone and weak, others are higher tone and overactive. A pelvic floor that struggles to fully relax can still leak, even if it feels “strong.”

• Timing and pressure management matter.
Leakage often occurs during activities that rapidly increase intra-abdominal pressure (e.g. coughing, lifting, running). This can reflect a coordination issue rather than strength alone.

• The pelvic floor is part of a system. It works alongside the diaphragm, abdominal muscles, hips and spine. Dysfunction elsewhere can influence pelvic floor symptoms.

• More exercises are not always the solution!
Doing pelvic floor exercises without assessment can be ineffective, or even make things worse.

This is why individual assessment is key. Treatment should be based on how your pelvic floor functions, not assumptions.

GUILTY 🚿

I know, I know… it feels efficient. But regularly peeing in the shower can actually train your bladder to associate the sound of running water with the urge to wee.

Over time, this can make your bladder over-reactive ➡️ hello sudden urgency and “I’ve got to go RIGHT NOW” feelings.

This habit can also contribute to urge incontinence, where leakage happens on the way to the toilet, because your bladder learns to empty before it’s truly full.

Your bladder loves clear signals. Wee sitting on the toilet, relax your pelvic floor, and let your bladder do its thing without background cues.

Many symptoms (like urgency, pain, leakage or heaviness) can sit in the overlap between weakness and overactivity.

A women’s health physio assessment looks at:
• strength and relaxation
• coordination and timing
• breathing, posture and load management
• your symptoms, goals and life stage

From there, treatment is tailored to you.

If something doesn’t feel right, trust that instinct.
