Dr Louis Wessels is a cosmetic, plastic and reconstructive surgeon and is a senior lecturer at the faculty of medicine and health sciences. His practice covers the whole range of practice in cosmetic, plastic and reconstructive surgery with special emphasis on cosmetic and reconstructive surgery of the breast (augmentation, reduction and breast lift) as well as facial cosmetic and reconstructive surgery. He is also an accomplished microsurgeon and regularly performs both reconstructive microsurgery for breast reconstruction and head & neck restoration following cancer surgery. In addition, he has expertise in body contouring surgery and non-surgical cosmetic therapies. Dr Wessels received his MBChB in South Africa, completing his internship & general surgery residency, training in one of the most challenging clinical environments, before completing his Fellowship and his Master of Medicine in Plastic & Reconstructive Surgery. He subsequently graduated from Macquarie University in the inaugural award of the Masters of Advanced Surgery (Plastic & Reconstructive Surgery), for which he was awarded a Scholarship from Johnson & Johnson. Dr Wessels successfully completed his second Fellowship in Plastic and Reconstructive Surgery when was awarded a Fellowship of the Royal Australasian College of Surgeons in 2015. He is a member of the Surgical Infection Research Group at Macquarie University, and has published world-leading research on breast implant infection and outcomes.

A breast augmentation & fat graft for a patient with tuberous breast anatomy.
Tuberous breasts are characterised by a tight lower pole, a high inframammary fold and a narrow breast base, often creating volume restriction and asymmetry. When the patient wants a surgical procedure it requires careful structural release, rebuilding of the lower pole and controlled enhancement of shape.
For this patient, we performed a breast augmentation with 440cc teardrop (anatomical) high-profile implants combined with strategic fat grafting.

This patient was an excellent candidate for a combined procedure of breast augmentation, breast lift and fat grafting, as she had minimal excess breast tissue and a moderate–good skin envelope, allowing us to safely achieve lift, volume restoration, and upper-pole shaping in one operation.

We placed 440cc round Moderate Plus Mentor implants to create balanced, natural fullness, and complemented this with fat grafting to the cleavage, using fat harvested from the inner thighs to refine contour and enhance softness over the implant.

Today’s result showcases a comprehensive revision procedure for a patient who had her implants in place for over 10 years. With time, both the breast tissues and implants naturally change, and she presented with concerns regarding shape, position, and overall aesthetic balance.
During surgery, we removed the original implants, performed a full breast lift to restore support and reposition the nipple–areolar complex, and placed new round 500cc Mentor implants to enhance upper-pole fullness and refine her proportions.

Revision surgery is highly individualised, and in cases like this, combining implant exchange with mastopexy allows us to correct age-related changes, improve symmetry, and achieve a more stable, long-lasting result.

We performed this surgery in two planned stages to ensure the safest and most refined result.
Stage One involved removing the patient’s existing implants and performing a breast lift, allowing the tissue time to heal, reshape and stabilise before introducing new volume.
Several months later, Stage Two focused on final contouring, a secondary lift for symmetry, placement of new implants, and targeted fat grafting to soften the upper pole and cleavage line. We used 370cc round Mentor implants.
While not required for every patient, a staged approach can offer greater precision, safety and predictability, leading to a more natural and long-lasting outcome.

A refined and anatomically natural result for this patient following breast augmentation with 440cc Mentor teardrop implants.
Teardrop implants allow me to recreate a gentle upper-pole slope and a more natural breast contour, particularly for patients seeking enhanced volume without a “rounded” upper fullness. In this case, the patient’s chest proportions, tissue characteristics and desired aesthetic made the 440cc anatomical implant the most appropriate choice.
Careful pocket creation and precise implant positioning are essential in achieving this level of symmetry and stability, and I’m very pleased with her outcome.

This patient underwent a planned staged approach to achieve the most stable, long-lasting outcome.
Stage 1: Mastopexy (breast lift) to reshape and elevate the breast, remove excess skin, and establish a stronger, more predictable breast footprint.
Stage 2 (6 months later): A secondary lift combined with breast augmentation using 425cc round high-profile implants to enhance upper-pole fullness and create a balanced, supported shape.
A two-stage strategy is often the safest and most effective option when:
➖The breast envelope is significantly stretched or ptotic, and needs to be tightened before supporting an implant
➖We need to reposition the nipple-areola complex accurately without the tension of an implant underneath
➖Skin quality or elasticity requires time to settle and contract before adding volume
➖The goal is longevity, reducing the risk of bottoming out, recurrent ptosis, or over-stretching the lower pole
➖We want to shape the breast first, then size the implant precisely once the new breast position has stabilised
By completing the lift first, we allow the tissue to heal, settle and mature.
