Hello, my name is Angela, and I am an NDIS Provider from Elevate Disability Care. Our organisation is committed to optimising social and economic independence and promoting full participation for people with disability. A core aspect of this vision is our dedication to becoming the preferred service provider within our community through high-quality, person-centred care. I am reaching out to express my interest in collaborating with you and supporting participants who require reliable and flexible NDIS services. Below is an overview of the comprehensive supports we provide
1. Core Supports - Assistance with daily activities to support independence and wellbeing, including daily living support, household tasks, community participation, and consumables.
2. Capacity Building Supports - Helping participants develop skills for long-term independence through Health & Therapy Services, Skill Development, and Support Coordination. Our Health & Therapy Services include Physiotherapy, Occupational Therapy, Speech Therapy, Psychology, and Dietetics. Skill Development encompasses life skills training, money management, travel training, and communication & social skills. Support Coordination involves understanding and implementing NDIS plans, connecting participants to appropriate services, and strengthening capacity to manage supports.
3. Capital Supports - Providing access to essential long-term equipment and modifications, including assistive technology and home modifications.
4. Specialist Supports - Behaviour support & behaviour plans, complex medical and NDIS nursing care, SDA, and SIL.
5. Plan Management - Processing and paying invoices, budget tracking and transparency, and flexibility to use registered and unregistered providers. With extensive experience in both hospital and NDIS settings, I ensure participants receive safe, compassionate, and professional support.
We are based in Eagleby 4207 and can take on Self-Managed and Plan-Managed participants across Brisbane, Logan, Ipswich, Caboolture, and the Gold Coast. If you or your participant is located outside these areas, please feel free to contact us - we are always happy to explore options to accommodate their needs.
Although we operate from a home-based office, we travel to participants to deliver services directly. We have passed our audit and are awaiting our registration number with the NDIS Commission.
Our operating hours are 24/7, and we are available to take on new participants today.
