Today’s deep dive is into
HUMAN BIOMECHANICS & the role of a :

~ podiatrists are experts in assessing the way the body moves
~ podiatrists closely analyse how people walk & run by examining the way the foot makes contact with the ground, and examining what occurs to the foot & every part of the body, both whilst the foot is in contact with the ground, and whilst it’s preparing to make contact

Understanding how a person moves, helps to inform why someone may have
- Pain
- Loss of function
- Areas of concern in the foot or lower leg

use many and varied strategies to offload vulnerable tissue where there is compromise to help
Relieve pain
Reduce symptoms
Strengthen weak areas
Restore loss of function.

Those strategies include
Taping
Padding (felt or foam)
Orthoses (prefabricated or customised)
Strength and conditioning
Massage
Changes to footwear
Biomechanical cues
Changes to activity.

“From first steps to next steps”

Yesterday we walked down the dermatology path with a look at corns & warts.
Today, we’re staying in that corner of podiatry to explore
INGROWN TOENAILS -

Ingrown toenails almost always occur at the big toenail (the nail of the hallux,) although it is possible for the toenail of a lesser digit to become ingrown.

At their most severe, an ingrown toenail may also be infected and there may be associated hypergranulation tissue (a vascular kind of tissue created by the body in response to the nail growing in.)
Ingrown toenails are almost always painful when left untreated.

HOW CAN WE HELP?
Sometimes, conservative care is all that’s required to address the problem, which is performed in clinic, with accompanying advice & education.

In most cases when the nail is ingrown and certainly when there is an infection present, we perform a simple procedure in the clinic called a nail resection or Partial Nail Avulsion (PNA.)
A PNA requires a local anaesthetic which we give you at the start, this makes the whole toe go numb 👏🏼
Then we remove the ingrown nail spike and phenolise the tissue- phenolisation is a process where we apply a chemical to the bottom of the nail to reduce the chance of it growing back.

We dress the area and provide advice and education on everything you need to know about the following day, few days, weeks and months ahead.

A PNA is simple and very effective.

Today we’re looking into DERMATOLOGY (the skin!)
And more specifically, the difference between a corn and a wart!

Let’s set the record straight:
✅ Both are treated by a
❌ They are definitely not the same thing

A corn is like a callous.
It’s an area of pressure, a build up of skin.
Always located over a weight bearing area, like a bony spot on the foot that touches the ground.

A wart is a virus.
The human papilloma virus that hides in the skin & has not been recognised by the body’s immune system to develop a line of defence.

Corns (in most cases,) come back over time, even after treatment.

Warts may return after being treated fully, but not always.

🦶🏽 Developmental delay
🦶🏽 The appearance of a “flat foot”
🦶🏽 Advice around footwear…just to name a few!

Many childhood presentations require advice, education, reassurance & a suggestion to monitor the child over weeks, months or years.
We’re here to put minds at ease.

In other cases, stretching, exercises, massage, assistance via walking cues, advice/changes to footwear, modifying activity and occasionally orthotic therapy, may form part of the treatment and management plan.

The big toe joint is a critical part of our anatomy and its a joint that has a big role to play in how we walk:
Its responsible for how we push-off the ground- we call that propulsion.
Sometimes though, a person might experience pain in the big toe joint and one cause of that pain could be the sesamoids.
They are small, egg shaped bones that form part of the joint. They are on the plantar aspect (bottom of the foot.)
It is critical than any pain- from mild symptoms to severe- loss of function, or change in sensation be assessed, diagnosed and managed by a podiatrist.
We need to respect and look after our big toe joints...they are very important!

Ankle joint injuries are common and just as they are common, they’re frustrating!
They may come on in an acute manner- dynamic movement causing a lateral ankle sprain;
Or the causal factor may be something more chronic- changes to ligament integrity and therefore function over time.
“Rolling out” so much that the outside of the ankle joint is compromised and becomes injured, is the most common method for injury and is known as a lateral ankle sprain.
Whether your ankle is symptomatic on the inside or the outside, perhaps it’s anteriorally (the front) or posteriorly (the back,) it’s important to have it assessed.
