Procedure: Removal and replacement bilateral breast implants + fat grafting.

Outcome: This patient has old breast implants which were malpositioned and starting to cause pain. I performed a removal of her breast implants and found that her old implants had rotated 90 degrees in her pocket. To improve the position of her implants I performed lateral closure of her capsule and medial dissection. I replaced and upsized her implants with a Motiva Ergonomix 300cc implant on the right and 265cc implant on the left. I used differential implant volumes to address her slight breast asymmetry. I also performed fat grafting to her upper medial right breast to further correct her asymmetry and improve her contour. Implants were supported with an internal bra. This is her immediate on table result showing an improved implant position and symmetry.

Surgery Duration: 1.5 hours
Hospital Admission: Day surgery
Recovery: Supportive bra at all time for 6 weeks.

An incredible experience learning and refining advanced facelift and facial aesthetic surgery techniques with Dr Giovanni Botti. I had the opportunity to learn the gliding brow lift, the high SMAS deep plane facelift, and a midface lift performed through a lower blepharoplasty approach. It was also a valuable chance to further refine my techniques including the classic deep plane facelift, platysmaplasty, and lip lift. Procedure: Immediate breast reconstruction following bilateral mastectomy. Two stage procedure with expanders (1st stage) and breast implants (2nd stage).

Outcome: This patient had a bilateral mastectomy for a triple negative breast cancer and was immediately reconstructed with Mentor 350cc tissue expanders placed under the muscle and an acellular dermal matrix (FlexHD). Her second stage procedure involved exchanging her expanders for definitive breast implants. Her expanders were initially sitting high and apart. I tell my patients to expect that the expander will often ride high and feel irregular, but at the time of the 2nd stage exchange I can manipulate the pocket to correct any malposition. In the 2nd stage, I corrected the implant pocket, especially to fill out the medial inferior pole of the left breast. The patient was upsized to Motiva 450cc ergonomix full profile implants. This is her result 4 weeks after the second stage procedure showing a lift and increase in her breast volume. Her breasts are sitting close together offering an excellent contour and shape. The implants are also a lot more comfortable than the expanders.

It’s been an awesome 2 days in Singapore learning from the best on Asian blepharoplasty. Was a great opportunity to see their approach and perform them on cadavers for non-incisional and incisional fold creation, ptosis correction, epicanthoplasty, lower blepharoplasty and canthopexy.

Procedure: Immediate breast reconstruction following bilateral mastectomy. Two stage procedure with expanders (1st stage) and breast implants (2nd stage).

Outcome: This patient had a locally advanced T2N1 left sided triple negative breast cancer. She underwent neoadjuvant chemotherapy and immunotherapy. A bilateral mastectomy and left sided sentinel lymph node was performed by General Surgeons where the left side mastectomy weight was 304g and right side was 286g. She was immediately reconstructed by me with Mentor 350cc tissue expanders placed under the muscle and an acellular dermal matrix (FlexHD). She was over expanded to 400cc. Her second stage procedure involved exchanging her expanders for definitive breast implants. The patient was upsized to Motiva 450cc ergonomix full profile implants. This is her immediate on table result.

Surgery 1st stage reconstruction: 2.5 hours
Surgery Duration 2nd stage reconstruction: 2 hours
Recovery: Supportive bra at all time for 6 weeks after each stage.

Procedure: Bilateral Breast Reduction

Outcome: Before and after photos at 5 months post-op. Breasts reduced to a volume proportionate to her body and asymmetry corrected. Breasts reshaped, and lifted to expose more torso. Breasts supported with an internal bra suture. The patient experienced improvement in her neck and upper back pain. Left side = 448g reduced. Right side = 442g reduced. Total = 890g.

Surgery Duration: 2.5 hours
Recovery: Supportive bra at all time for 6 weeks.

Operation: Excision of lesion nasal tip + full thickness skin graft. Before and after taken 1 month post-operative.

Outcome: This patient had an aggressive basal cell carcinoma (BCC) to his nasal tip which required an excision with a wide margin. I reconstructed it with a full thickness skin graft from his glabella/forehead. I prefer harvesting a full thickness skin graft from this region because the colour and thickness of the skin matches the nasal tip the best. I am careful to put the scar in existing vertical frown lines. His graft has healed well and his forehead scar has been placed in aesthetic lines. Pay attention to the colour match and contour of his nasal tip reconstruction, even being 1 month post-operative.

Surgery Duration: 30 minutes
Hospital Stay: Day surgery
Anaesthetic: Twilight sedation + local anaesthetic

I’ve just finished a rewarding 4 days in Cairns as a plastic surgery locum. It was a privilege to work alongside such a dedicated team of nurses, theatre assistants, anaesthetics and plastic surgery registrars/residents. Over the course of 3 days we operated on 33 patients, excising 46 aggressive skin cancers. We also managed to review 38 new patients in clinic to have their skin cancer managed. I’m grateful for the opportunity to be part of such excellent collaborative care for the Cairns community. I am also grateful to have been able to sneak a swim and run in 🏊🏼‍♂️🏃😉

Operation: Excision of soft tissue tumour on right ring finger and pulp flap reconstruction. Before and after taken at 2 month post-operative.

Outcome: This patient presented with a slow growing soft tissue tumour in the pulp of her right ring finger. She is right handed and is an artist. This tumour occupied almost 100% of her finger tip. Direct excision would have meant thin pulp skin and scar directly over her bone, resulting in a sensitive and empty finger tip. I performed an excision of the tumour, preserving the small nerves and blood vessels to her finger tip. Then I reconstructed the finger with a buried pulp flap to give her finger tip volume. I also offset her scar to the non-dominant side of her ring finger. She has healed well and is happily back painting.
