Operation: Excision of soft tissue tumour on right ring finger and pulp flap reconstruction. Before and after taken at 2 month post-operative.

Outcome: This patient presented with a slow growing soft tissue tumour in the pulp of her right ring finger. She is right handed and is an artist. This tumour occupied almost 100% of her finger tip. Direct excision would have meant thin pulp skin and scar directly over her bone, resulting in a sensitive and empty finger tip. I performed an excision of the tumour, preserving the small nerves and blood vessels to her finger tip. Then I reconstructed the finger with a buried pulp flap to give her finger tip volume. I also offset her scar to the non-dominant side of her ring finger. She has healed well and is happily back painting.

What is my approach to plastic surgery?

I take an artistic approach to my operating. Rather than relying on a one-size-fits-all (recipe) operation, I carefully analyse your anatomy and marry it up with your preferences and goals. I then apply a set of techniques to create a result that is tailored specifically to you.

Sometimes this means an operation takes longer, or that I redo components of a procedure that I am unhappy with. But I’m not afraid to do that because every operation should be customised to your goals, desires and unique anatomy. No two patients are the same.

Breast asymmetry is normal - in fact everyone has some degree of difference between their breasts.

When I perform breast surgery, I always strive to improve any asymmetry. But it is important to remember that perfect symmetry is impossible.

As I often tell my patients - “I’m trying to make sisters, not twins.”

Operation: Bilateral upper eyelid blepharoplasty and ptosis repair with levator advancement.

Outcome: This patient had moderate upper eyelid ptosis (drooping eyelid) and excess upper eyelid skin causing dermatochalasis. This was causing significant visual field obstruction for the patient to the point where she had to open her eyelids with her fingers to watch TV. I performed an upper blepharoplasty/double eyelid surgery to address her excess upper eyelid skin. I also performed an anterior ptosis repair where I advanced her upper eyelid levator muscle to address her drooping eyelid. I also excised a xanthalasma to her left upper eyelid. This is a before and after taken immediately post-operative, 1 week post-operative and 8 weeks post-operative. Pay attention to her improved upper eyelid position and shape, as well as her well defined upper eyelid crease. She had an excellent outcome, where she can now watch TV without using her fingers to open her eyelids.

Surgery Duration: 60 minutes
Hospital Stay: Day surgery
Anaesthetic: Twilight sedation + local anaesthetic

Operation: Excision of lesion right medial cheek + cheek advancement flap. Before and immediate on table result.

Outcome: This patient had a basal cell carcinoma (BCC) to her right medial cheek. I reconstructed it with an advancement flap from the cheek. This advancement flap has the distinct benefit over a VY island flap because it prevents any pincushioning effect. Her skin cancer was excised with clear margins. Pay attention to the position of the lower eyelid which has been maintained and supported with the advancement flap.

Surgery Duration: 30 minutes
Hospital Stay: Day surgery
Anaesthetic: Twilight sedation + local anaesthetic

Skin Cancer Masterclass
It was a pleasure to teach at this weekend’s skin cancer masterclass hosted by , alongside my colleagues and .

In plastic surgery, there is no one-size-fits-all approach. Every reconstruction should be guided by the core principles of restoring both form and function, tailored to the individual patient, rather than a prescribed formula. There is an art to what we do, and it was a privilege to share the nuances, techniques, and insights gained through training and clinical experience in my talk.

Workshops like these are a valuable opportunity to collaborate with GPs, nurses, and medical students. They strengthen our professional ties and, most importantly, help deliver better care to our shared patients.

Operation: Excision of lesion right nasal side wall + nasalis advancement flap. Before and after taken at 8 weeks post-operative.

Outcome: This patient had basal cell carcinoma (BCC) to his right nasal side wall which required an excision with a wide margin. I reconstructed it with an advancement flap based on his nasalis muscle. This advancement flap has the distinct benefit over a VY island flap because it prevents any pincushioning effect. His skin cancer was excised with clear margins. His flap has healed well and his scars have been placed in aesthetic lines. Pay attention to the colour match and contour of his nasal reconstruction.
