Addiction Support Worker
Habitat is a residential drug and alcohol rehabilitation service based in Geelong, Victoria, we service clients from across Australia. We are licenced as a Private Hospital with Dept. Human and Health Services, Victoria and accredited to conduct Alcohol and Drug Detoxification.

POSITION PURPOSE
The Support Worker contributes to the provision of a safe and hospitable environment for clients voluntarily seeking withdrawal from the acute use of alcohol and/or other drugs by actively supporting clients to achieve their individual treatment plans including drug withdrawal, in collaboration with counsellors, the case managers and the medical team.
Work collaboratively with AOD Support workers, case managers, counsellors and the medical team.
KEY RESPONSIBILITY AREAS
Individual Treatment Plans
Communicate and build supportive relationships with clients to assist them to achieve the goals identified in their individual treatment plans.
Review with clients their progress in relation to their individual treatment plan and discuss with the Counsellor, Case Manager, Clinical staff and other professionals as required.
Provide high quality, evidence-based withdrawal services to assist clients cease their problematic use of alcohol or other.
Work with clients to review and implement their Individual Treatment plans, developed at assessment.
Provide ongoing monitoring and observations of clients including close liaison with General Practitioners/Nurses, monitoring of withdrawal symptoms, mental health screening and assessment and documentation of progress.
Provide education and information through individual and group-based programs as part of the overall program and encourage clients to participate fully.
Take clients to appointments in the community or at other services as required.
Ensure that collaborative and productive working relationships with current and potential key stakeholders are established and maintained.
Qualifications:
A relevant qualification in a health and/or welfare related discipline.
Certificate IV in AOD or Alcohol and Other Drug core competencies (or willingness to complete the core competencies within the first 12 months of employment).
Demonstrated ability to effectively work with clients and families experiencing problematic alcohol and other drugs use in a respectful, compassionate and positive manner.
