Over the weekend I saw a post in a running group that really stuck with me. A runner had just been diagnosed with chondromalacia and was heartbroken to be told that running might no longer be the best option for her. She was asking if anyone else had been through the same thing and what had worked for them.
The comments that followed were incredible. Dozens of people shared their own stories of being diagnosed with the same condition, and many said they had found their way back to running. The common theme was strength training guided by a physio or exercise physiologist who knew how to manage the condition.
➡️What is chondromalacia?
It happens when the cartilage under the kneecap softens and becomes irritated, leading to pain at the front of the knee. It is one of the most common causes of knee pain in runners, affecting up to 30% of them at some stage.
➡️The hopeful part
It does not have to mean the end of running. With the right strength program, focusing on the muscles around the hips, quads, and glutes, many runners reduce pain, improve knee function, and return to the sport they love.

We recently saw a 21-year-old female who had been given the all clear by her onsite workplace doctor to return to full duties after a grade 2 calf strain.
Her rehab had been minimal, mostly rest and avoiding leg exercises. She was cleared based on her original ultrasound scan and completing some basic movements: calf raises, hopping, jogging and a quick sprint, all of which she did well in front of the doctor.
Her job is physical and requires sprinting, changing direction and being on her feet most of the day. Before her first shift back, she wanted to do a test run to see how her leg felt. She admitted she started the run pretty quickly and with minimal warm-up. Unfortunately, she re-strained her calf during that run.
This is why informal clearance tests after a period of rest are not enough. At Adjust, when we see people with acute injuries that are likely to need high-demand return, we don’t stop at scan results or simple movement screens. All our clients follow a progressive, loaded rehabilitation program with objective testing throughout the stages of healing. This is how we get better long-term outcomes.
Proper rehab means gradually building calf strength and power, and using force plates and measurable calf strength/RFD tests to know when the tissue is truly ready for full load again.
This setback could have been avoided with good early management. Now she’ll need another full block of rehab before returning to her physical role safely, which we are excited to help her with 💚

At Adjust Physio in Oxenford, we often see people for rehabilitation after surgery, helping them rebuild strength, movement and confidence as they recover.
But what many people don’t realise is that the work you do before surgery can make just as much of a difference to how well you bounce back.
When we see people with acute injuries that are likely to need surgery, our treatment doesn’t stop after the scan results. We get straight into prehabilitation because we know it gets great results in the long term.
A meta-analysis of 48 trials with more than 3,500 patients found that prehabilitation before orthopaedic surgery significantly improves function and strength, and leads to better function after surgery compared with usual care.
Going into surgery stronger helps you come out stronger.
