What is Delegation of Care & Care Planning? 🤔

This is necessary (as per the NDIS standards: skills descriptors ✅) to effectively support participants who need their support workers (SW) trained & guided with their care requirements (typically for high intensity supports/ADLs) & *where there is not a RN on staff*.

The NDIS funds participants (via their plan) for RN oversight & for the RN to essentially provide training & guidance, 1:1 or with the team, & sometimes with the participant present.
This service provides a RN on-call during business hours & the RN uses report writing time to write the care plan (& provide training for SWs on the care plan) to follow when providing supports. It is commonly related to High Intensity Supports and tasks such as ADLs.

Why might I need this service? 💡
*You're a support coordinator & your preferred provider doesn't have capacity due to insufficient staff training.
*You’ve received a referral for complex supports & your team needs help with care planning, service delivery & training. 📃
*You don’t have a RN on staff.
*You have an Audit pending & require RN oversight?
*Need to access ongoing RN support?
*Need individual or group training for high intensity supports 🛗? Manual Handling? 🧑‍🦽‍➡️

QLD Health Academy Care Service (aka QHACS) has been supporting individuals to achieve optimal health outcomes, since 2019. We have a team of >70 nurses who are diligent in providing care and support, whilst facilitating independence.

We provide Community Nursing between the Gold Coast/Brisbane/Moreton Bay Regions, consisting of a wide variety of nursing care.

Did you know we are also a Specialist Provider of High Intensity Supports and Specialist Community Nursing? QLD Health Academy Care Services' (QHACS)

QHACS specialises in supporting individuals who are medically complex and would be best suited to supports delivered by a qualified Nurse. 99% of our support staff are Nurses with hospital/community/aged care/mental health/lived experience &/or forensic backgrounds.

We aren't a Yes provider; we are ethical in that we assess every referral individually to determine if we are the best provider for the participant and vice versa. If we can't meet the needs of the participant, we make recommendations of other providers, for the participant to decide their next step, supporting choice and control.
