Do you have an Ingrown toenail?

An ingrown toenail is where the sides of the toenail are curved and grow into the skin which can cause pain and discomfort.

If an ingrown toenail progresses it can pe*****te the skin which can eventually lead to an infection.

When to seek treatment:

Pain when pressure is applied to the toe
Redness
Warmth
Swelling
Pus

Possible Treatment Options

Conservative management which may simply involve carefully removing the toenail without the need for any pain relief. This is often only a temporary measure and does not necessarily permanently fix the issue.

Onyfix (nail bracing). The brace is applied to the toenail and remains on the nail for a period of time. As the nail grows out the shape of the nail is corrected. The process is painless.

Surgical. This is typically recommended in cases where the problem is severe or when conservative treatment is deemed ineffective. Nail surgery is a specialty procedure which may involve removing part or all of the toenail.

Any surgical or invasive procedure carries risks. Before proceeding, you should seek a second opinion from an appropriately qualified health practitioner.

Approximately 1 in 3 people (over the ages of 50) with diabetes have Peripheral Arterial Disease (PAD). PAD is where there is reduced blood flow to the legs and feet caused by blocked or narrowed arteries. This can lead to poor wound healing and increase the risk of infections.

A podiatrist can help detect if your circulation is compromised. If there is any suspicion of abnormal blood flow, we will often communicate with your GP to conduct further investigation.

Complications which can occur from reduced blood flow include foot and lower limb infections such as cellulitis or bone infections, delayed wound healing and foot ulcers.

Symptoms

β€’ Intermittent pain in the calf muscle area (cramping)
β€’ Worsening pain during exercise (usually in the legs)
β€’ Easing of pain during rest (usually in the legs)
β€’ Coldness of the affected body part
β€’ Numbness
β€’ Pins and needles
β€’ Muscular weakness
β€’ Blue or purple tinge to the skin
β€’ Wounds that won’t heal (vascular ulcers)
β€’ Blackened areas of skin or skin loss (gangrene).

Suggestions from a podiatry assessment may include:

β€’ Education pertaining to the prevention of foot complications
β€’ 6-12 monthly assessments depending on symptoms and severity
β€’ Continued nail and skin care to address and nail or skin conditions
β€’ Footwear advice and suggestions

Things to remember:

β€’ PAD is due to narrowing or blockages in the arteries
β€’ Risk factors include diabetes, obesity, smoking and a sedentary lifestyle.
β€’ Contact your GP or Podiatrist if you see any signs of redness, warmth or swelling

80% of diabetes-related amputations are preventable with proper treatment and ongoing management.

Regular diabetic foot screenings by a podiatrist is very important. Diabetes can lead to foot and lower limb complications which can significantly impact one’s quality of life. A podiatrist can help assess and monitor any potential issues that may arise.

Effective podiatric intervention can drastically improve diabetic foot outcomes. Successful diabetic foot management will involve diabetic foot screening, staging the diabetic foot, modifying risk factors, preventative foot care and treatment of ulcers.

Regularly inspecting diabetic feet and taking immediate action when a concern arises is the key to preventing a major problem. All people with diabetes should have their feet assessed at least once every 12 months.

The aims of a diabetic foot examination are to:

β€’ Evaluate and classify the foot
β€’ Detect any serious problems
β€’ Provide education on foot care and prevention
β€’ Act upon any findings
β€’ Refer on if necessary

During the assessment we perform a number of important tests which will help determine if your feet are low or high risk. We will also be able to advise on how frequently you need your feet checked and if any treatment or ongoing care is required.

Ingrown toenails

An ingrown toenail is where the sides of the toenail are curved and grow into the skin which can cause pain and discomfort.

Symptoms

β€’ Pain when pressure is applied to the toe

Treatment

We treat ingrown toenails conservatively and surgically depending on the severity.

Non-Operational

Conservative management may simply involve carefully removing the toenail using special instrumentation. There is usually little to no pain.

In certain cases, we can also use a nail correction system called Onyfix. The product is applied to the toenail and remains on the nail for a period. As the nail grows out the shape of the nail is corrected. The product will remain on the toenail for up to 3 months and is painless.

Operational

Surgery is typically recommended in cases where the problem is severe or when conservative treatment is deemed ineffective. The procedure is performed under local anaesthesia so you will feel no pain. Post-operative pain is either absent or minor.

Nail surgery is a specialty procedure which may involve removing part or all of the toenail. This procedure is usually recommended in acutely severe or chronic cases where conservative treatment has been unsuccessful or is no longer feasible.

Any surgical or invasive procedure carries risks. Before proceeding, you should seek a second opinion from an appropriately qualified health practitioner.

Warts are a common contagious infection in the outer layers of the skin which are caused by HPV (Human Papillomavirus). They can spread to other areas of our body and from person to person. Warts can be difficult to get rid of as they are excellent at evading the body’s immune system.

Signs and Symptoms

- A small, fleshy, rough, grainy growth (lesion) on the bottom of you foot

- Hard, thickened skin (callus) over a well-defined "spot" on the skin

- Black pinpoints, which are commonly called wart seeds but are actually small, clotted blood vessels

- Pain or tenderness when walking or standing

When to see a Podiatrist

- The lesion is bleeding, painful or changes in appearance or colour

- You've tried treating the wart, but it persists, multiplies or recurs

- Pain interferes with activities

- You also have diabetes or poor sensation in your feet

- You also have a weakened immune system because of immune-suppressing drugs or an immune system disorder

- You aren't sure whether the lesion is a wart

Fungal nail infections will typically cause toenails to become thickened, brittle, and discoloured.

Fungal infections tend to affect toenails more than fingernails due to their slower rate of growth, reduced blood supply and being confined to footwear.

Causes

Onychomycosis can be due to infection with dermatophytes or non-dermatophytes such as moulds and yeasts.

Dermatophyte fungi are the ringworm fungi (tinea) and these account for over 75% of cases.

Non-dermatophytes are basically moulds and yeasts and occur in about 10% of cases.

Treatment

Mild infections affecting < 50% of one or two nails may respond to topical therapies and routine nail care such as Vicks Vaporub or over-the-counter (OTC) treatments purchased from a pharmacy.

Most treatment regimes will take a minimum of at least 6-12 months and will require regular nail debridement.

It is often recommended that nail tinctures should be combined with other topical antifungal therapies such as terbinafine hydrochloride. This is because these types of fungal infections are often present in the skin as well as the toenails.

If there is total nail involvement or if multiple toenails are involved, then the following treatment options may be advised:

Oral antifungal medication, however, you will be required to be on this treatment for several months. This medication is not suitable for everyone due to drug interactions and potential liver injury.

Removal of the nail. In certain cases, the toenail will need to be removed for testing purposes and to help facilitate a new nail.

Cold Laser. Is a safe and painless treatment (non-thermal) that increases blood flow to the area which aids the immune response to toenail infections.

Hot Laser. A thermal laser which uses an infrared beam to target and heat up fungal spores until they can no longer survive. Patients may experience pain with this treatment.
