We are currently seeking a motivated and experienced Specialist Support Coordinator to join our growing team on a part-time basis. This role is ideal for a professional with strong knowledge of the NDIS and experience supporting participants with complex needs.
About the Role
As a Specialist Support Coordinator, you will support NDIS participants with complex needs, helping them navigate the NDIS, overcome barriers, and coordinate services that promote long-term capacity building and independence.
You will work collaboratively with participants, families, support networks, and service providers to ensure supports are effectively implemented and aligned with participant goals.
Key Responsibilities
Deliver Specialist Support Coordination in line with NDIS guidelines and best practice
Assist participants with complex needs to understand and implement their NDIS plans
Coordinate and monitor a range of services and supports to ensure they are meeting participant goals
Develop strong relationships with participants, families, carers, and stakeholders
Address barriers and complex challenges impacting participants' support arrangements
Facilitate service agreements and collaboration between multiple providers
Maintain accurate case notes, documentation, and reports
Advocate for participant rights, choice, and control
Identify risks and implement appropriate safeguarding measures
Support participants to build capacity and independence
Essential Qualifications & Requirements
Relevant tertiary qualification in Social Work, Psychology, Disability, Community Services, Mental Health, or a related field
Demonstrated experience providing NDIS Support Coordination or Specialist Support Coordination
Strong understanding of the NDIS framework, legislation, and practice standards
Experience supporting individuals with complex support needs including psychosocial disability, behavioural challenges, or multiple support systems
Excellent case management and problem-solving skills
Strong interpersonal, communication, and advocacy skills
Ability to work independently while collaborating with multidisciplinary teams
High level of organisational and documentation skills
Proficient computer skills and experience using client management systems
Mandatory Checks & Compliance
Current NDIS Worker Screening Check
Working With Children Check (or equivalent)
National Police Check
Current First Aid & CPR Certificate
Valid Driverβs Licence and access to a reliable vehicle
Right to work in Australia
Desirable
Experience working with high-complexity participants
Knowledge of NDIS Quality & Safeguards Commission requirements
Experience working with SIL, SDA, psychosocial disability, or behavioural support frameworks
