These assessments help you:

SEMPHN’s Vulnerable Peoples Homebound Vaccination Program has been extended to 30 June 2026, making COVID-19 immunisation more accessible. Anyone can request a home visit for an eligible individual, and you can also self-refer.

New statewide referral criteria are now in effect for children’s surgery and children’s ophthalmology conditions in public hospitals.

The Victorian Department of Health has developed and published statewide referral criteria for 9 children's surgery conditions and 8 children’s ophthalmology conditions.

The aim of statewide referral criteria is to improve access to public hospitals in Victoria by providing clear and concise guidance on when a referral for specialist opinion and management is indicated.

Applications are open for the Victorian Government-funded General Practice Innovation Grant Program. Eligible practices may apply for grants of $45,000 to implement an innovative model of care for the early identification and proactive management of dementia and mild cognitive impairment over a 12-month period. Applications close on 16 December 2025.
