This is the official page for South Eastern Melbourne PHN (SEMPHN). SEMPHN works on behalf of the Australian Government to improve health care for the benefit of 1.6 million people living in our region. We want everyone in our community live their healthiest lives. We support this by providing:

- Evidence, planning and influencing services to meet population health needs
- Capacity building services to grow primary health and its workforce in our region
- Commissioning services to translate national and state policy into local services and the regional health
system. Consumers, clinicians and other providers are integral to our efforts to improve local health care. Follow and engage with us to help make health care better.

Accidents can happen during the happiest times. If you or a loved one has an unexpected injury that needs medical attention on the same day, that’s a case for a Medicare Urgent Care Clinic.
Things that may need urgent care include minor fractures, sprains, neck and back pain, mild burns and minor cuts.

Medicare Urgent Care Clinics are:

🎄Bulk billed, so services are free
🎄Walk-in, so you don’t need an appointment or referral
🎄Open early and late every day over the holiday season
🎄Staffed by GPs and nurses.

Do you know someone who would benefit from receiving a COVID-19 vaccination at home? SEMPHN’s Vulnerable Peoples Homebound Vaccination Program has been extended to 30 June 2026, making COVID-19 immunisation more accessible. Anyone can request a home visit for an eligible individual, and you can also self-refer.

New statewide referral criteria are now in effect for children’s surgery and children’s ophthalmology conditions in public hospitals.

The Victorian Department of Health has developed and published statewide referral criteria for 9 children's surgery conditions and 8 children’s ophthalmology conditions.

The aim of statewide referral criteria is to improve access to public hospitals in Victoria by providing clear and concise guidance on when a referral for specialist opinion and management is indicated.

Eligible practices may apply for grants of $45,000 to implement an innovative model of care for the early identification and proactive management of dementia and mild cognitive impairment over a 12-month period.
