The factsheet provides clear, practical guidance on how advance care planning can be incorporated into routine processes and clinical care, supported by existing MBS items, including:
👉Health assessments
👉GP chronic condition management plan preparation and review
👉Time‑tiered general attendances.

"We can't know anyone else's mind.
We need their view"

When Mark, a young man with MND, presented to hospital in crisis, the medical teams saw a young man with rapidly-progressing loss of muscle function affecting mobility, speech, swallow & breathing. They feared intervention would condemn him to a life of insufferably constricted horizons: bedbound, unable to communicate. But Mark’s family knew him best.

Mark's story, written entirely using his eye-gaze technology is an excellent read, and is highlighted below by Dr Kathryn Mannix. It's an important example of 'your story, your choice', and well worth a Monday afternoon read!

On the road and making a difference 🚗💬

It’s great to see our Education Coordinator Kylie Ash out across the ACT and NSW this week, supporting aged care providers to strengthen best‑practice advance care planning. From Canberra to Goulburn and the Illawarra, these workshops reflect the strong commitment of the aged care workforce to person‑centred care.

A big thank you to PHN ACT and Capital Health Network for supporting the Canberra workshop, to RSL LifeCare for enabling staff to attend, and to Warrigal Aged Care for rounding out the week with three engaging sessions. We also value the opportunity to collaborate with local leaders like ACT ACP Manager Janine Rath.

As we head into the Easter break, we wish everyone a wonderful long weekend. Travel safely, take care, and thank you to all those working over the holiday period to support older Australians and their families. 🐣💛

What an incredible Advance Care Planning Week 2026 💙

With over 200 activities held across Australia, this year’s ACP Week was our biggest yet. From Zumba classes, farmers’ expos, death cafés and pancake afternoon teas to book clubs, workshops and library displays, Australians showed up with creativity, care and a shared commitment to planning ahead for their health care.

Coming off the back of Advance Care Planning Week, it was timely to continue the conversation and bring advance care planning to the agenda, especially in the context of supporting older Australians and those living with frailty.

A big thank you to the organisers, facilitators and consumer representatives who brought such rich insight and lived experience to the discussion. These conversations are vital in strengthening person centred care, ensuring individuals’ values and wishes guide their health care across all stages of life.

We’re grateful for the opportunity to contribute and collaborate in shaping better outcomes for communities across Australia.
