Your story is yours to tell, and your healthcare choices deserve to be heard.

This National Advance Care Planning Week, 16-22 March 2026, Advance Care Planning Australia invites all Australians to pause and reflect on what matters most, as we embrace the theme ‘Your story, your choice.’

Every story is unique. We’re encouraging people to share their experiences and decisions, to think, talk and plan for the care they’d want in the future. To bring this theme to life, Advance Care Planning Australia is capturing powerful personal stories that will feature throughout the 2026 campaign.

Stay tuned, campaign resources will be available early next year to help you raise awareness and spark meaningful conversations across your organisation and community networks.

What do aged care providers need to do to reflect a rights-based approach to advance care planning?

As of 1 November 2025, the new Aged Care Act 2024 (Cth) and Strengthened Aged Care Quality Standards came into effect, embedding a rights-based approach at the heart of aged care.

With the introduction of the new Statement of Rights, older people receiving aged care are now legally recognised as rights holders. Core principles such as independence, autonomy, empowerment, and freedom of choice are central to the Statement of Rights, and perfectly aligned with the goals of advance care planning.
