What is Degenerative Disc Disease?
By Peter F. Ullrich, Jr., MD

Degenerative disc disease is one of the most common causes of low back pain, and also one of the most misunderstood.

Degenerative Disc Disease is a Misnomer:

A large part of many patients’ confusion is that the term “degenerative disc disease” sounds like a progressive, very threatening condition. However, this condition is not strictly degenerative and is not really a disease:
• Part of the confusion probably comes from the term "degenerative", which implies to most people that the symptoms will get worse with age. The term applies to the disc degenerating, but does not apply to the symptoms. While it is true that the disc degeneration is likely to progress over time, the low back pain from degenerative disc disease usually does not get worse and in fact usually gets better over time.
• Another source of confusion is probably created by the term "disease", which is actually a misnomer. Degenerative disc disease is not really a disease at all, but rather a degenerative condition that at times can produce pain from a damaged disc.

Disc degeneration is a natural part of aging and over time all people will exhibit changes in their discs consistent with a greater or lesser degree of degeneration. However, not all people will develop symptoms. In fact, degenerative disc disease is quite variable in its nature and severity.

Medical Practitioners Disagree on Degenerative Disc Disease:
Finally, many patients are confused about degenerative disc disease because many medical professionals don’t agree on what the phrase describes.

In practical terms, this means that few practitioners agree on what does and does not constitute a diagnosis of degenerative disc disease. Even medical textbooks don’t usually attempt to give an accurate description. Therefore, while many practitioners believe that degenerative disc disease is a common cause of low back pain in young adults, very few agree on the implications.

While there is still a lot of debate in the medical community about degenerative disc disease, a few aspects of the condition are known. This article will discuss aspects of degenerative disc disease that are more commonly accepted, such as the theory of the degenerative cascade, as well as some areas of theory that are still a source of debate in the medical community.

Pain from Degenerative Disc Disease:
The lumbar disc is a unique and well-designed structure in the spine. It is strong enough to resist terrific forces in multiple different planes of motion, yet it is still very mobile. The disc has several functions, including acting as a shock absorber between the vertebral bodies.

The lumbar disc has been likened to a jelly donut. It is comprised of a series of bands that form a tough outer layer and soft, jelly-like material contained within:

Annulus fibrosus—the disc’s firm, tough outer layerNerves to the disc space only pe*****te into the very outer portion of the annulus fibrosus. Even though there is little innervation to the disc, it can become a significant source of back pain if a tear in the annulus reaches the outer portion and the nerves become sensitized. With continued degeneration, the nerves on the periphery of the disc will actually grow further into the disc space and become a source of pain.

Nucleus pulposus—the jelly-like inner disc materialThe nucleus pulposus contains a great deal of very inflammatory proteins. If this inner disc material comes in contact with a nerve root, it will inflame the nerve root and create pain down the leg (sciatica or radiculopathy). In the same manner, if any of the inflammatory proteins within the disc space leak out to the outer annulus and touch the pain fibers in this area, it can create a lot of low back pain.

Source of the Pain:

Generally, the pain associated with degenerative disc disease is thought to stem from two different factors:
• Inflammation
• Abnormal micromotion instability

The proteins in the disc space can cause a lot of inflammation, and inflammation in the disc space can lead to low back pain radiating to the hips. The associated pain can also travel down the back of the legs.

If the annulus—the outer rings of the intervertebral disc - becomes damaged or worn down, it is not as effective in resisting motion in the spine. This condition has been termed “micromotion” instability because it is usually not associated with gross instability (such as a slipped vertebral body or spondylolisthesis).

Both the inflammation and micromotion instability can cause muscular spasm in the low back. The muscle spasm is the body’s attempt to stabilize the low back. It is a reflex, and although the body’s response of muscle spasm is not necessary for the safety of the nerve roots, it can be quite painful.

Nature demands a lot from Women.

By assigning their bodies the overwhelming burden of responsibility for the task of reproduction, we ask significantly more of Women's bodies than we ever do of Men's.

Men's bodies are tasked with only one obligation, to sustain the life of the host.

Women's bodies are tasked with creating it.
..then nurturing this new life when it cannot possibly sustain itself, with providing anything this new life needs, to growing it, and protecting it... then, and only then, will it commit to the task of sustaining itself.

By merely possessing a reproductive system, a woman is more prone to chronic diseases than men. They have seven unique organs that men don't, all committed to the reproductive process. Which means more organs that must be developed, cared for, and can at any point become cancerous or malfunction.

Menstrual cycles are at best a monthly inconvenience for most of their lives, and at worst an endless excruciating physical torture from within their own bodies... and that's all before the physical duress of carrying a child.

During pregnancy a Woman's body is twisted and stretched in a way no Man's could ever endure, their organs are squashed, its nutrients and vitamins are diverted, often with months of debilitating nausea and fatigue, and at least a few weeks of almost complete incapacitation.

Only then to be compelled to endure hours of anatomically defying physical trauma which often literally tears through her skin. Or perhaps she is afforded the comparably less glorified pain of invasive surgery being conducted on her WHILE SHE'S AWAKE!!!

Over the course of human history, more Women have died in child birth than all the Men that have ever died in all of history's wars.

By bearing humanity's children, we ask a lot of Women.
..all they ask for in return is the right to choose when they do it.

Every woman should have that right.

DEAR DAUGHTER

I hope with all my heart that I showed you the real me.
That I didn’t pretend I had it all together, or that life was not hard.
I hope I gave you the belief of you, in your core.
That I loved you enough, albeit messily, to code a blueprint for life.
To show you what love should look like.
And I hope I let you see me break, so you could understand, it is not an ending, rather a step.
And it’s vital.
Dear Daughter, I could not possibly have gotten everything right, and perhaps, thats the best thing I have given you.
That knowledge. No one gets it right.
We are not here to be perfect, we are here to love, to grow stronger and more bright with every generation.
Grow brighter my love, brighter than me.
As it very much should be.
And when I can no longer be with you, remember, my cells live within you.
You cannot, ever, lose me.
Not really.
We are a deal, a two for one.
A team.
For life.
And everything after that.

The Little Girl Who Couldn’t Sit Still
“Gillian is a seven-year-old girl who cannot sit in school. She continually gets up, gets distracted, flies with thoughts, and doesn't follow lessons.

Her teachers worry about her, punish her, scold her, reward the few times that she is attentive, but nothing.

Gillian does not know how to sit and cannot be attentive. When she comes home, her mother punishes her too. So not only does she have bad grades and punishment at school, but she also suffers from them at home.

One day, Gillian's mother is called to school. She is sad as someone waiting for bad news, takes her daughter’s hand and goes to the interview room. The teachers speak of illness, of an obvious disorder. Maybe it's hyperactivity or maybe she needs a medication.

During the interview an old teacher arrives who knows the little girl. He asks all the adults, mother and colleagues, to follow him into an adjoining room from where she can still be seen. As he leaves, he tells Gillian that they will be back soon and turns on an old radio with music.

As the girl is alone in the room, she immediately gets up and begins to move up and down chasing the music in the air with her feet and her heart. The teacher smiles as the colleagues and the mother look at him between confused and compassionate, as is often done with the old. So he says: "See? Gillian is not sick, Gillian is a dancer!"

He recommends that her mother take her to a dance class and that her colleagues make her dance from time to time. She attends her first lesson and when she gets home she tells her mother: "Everyone is like me, no one can sit there!"

In 1981, after a career as a dancer, opening her own dance academy and receiving international recognition for her art, Gillian Lynne became the choreographer of the musical "Cats."

Hopefully all “different” children find adults capable of welcoming them for who they are and not for what they lack ”.

Long live the differences, the little black sheep and the misunderstood. They are the ones who create beauty in this world..
