Rick arrived at Ozanam House after being discharged from The Royal Melbourne Hospital.

A project manager in the construction industry, he once inspected homes and commercial buildings. Now, he doesn’t have a home of his own. No family to call.

He carries a hospital discharge plan in his hands and a heavy bag on his back, too heavy, given the Angina that tightens his chest with every step. Two blockages in his heart. Ten kilos lost in just a few months, without trying.

Every daily task feels like a battle, and his biggest fear? His medications. He can’t afford them.

After being discharged, Rick had no money to see a GP or even fill his scripts. Like so many others, he was left in a gap, too unwell to manage alone, and without the means to get help. The weight of that uncertainty sat on his chest, just like his heart condition.

This is where the health services at VincentCare - Ozanam House and the Mobile GP service stepped in. Through this service, Rick was able to see a telehealth doctor from Atticus Health, get his medications reviewed, and in a flash he felt like he was regaining some control over his health.

Ozanam House paid for his medications and arranged for them to be collected from North Melbourne Pharmacy.

As soon as Rick received his eScripts, he exhaled, finally able to breathe, even if just for a moment.

He quit smoking five years ago, thinking he was making a choice for a better future. But today, that future feels uncertain. His financial situation is dire. He’s looking for work, but with his health in crisis, the odds feel stacked against him. He feels sad, depressed, anxious, trapped in a cycle he can’t seem to break.

And yet, Rick still shows up.

The strongest people aren’t the ones at the top.

They’re the ones who refuse to let rock bottom define them.

For many of us, a simple text or calendar alert is all it takes to remember an appointment.

But what if you don’t have a phone? What if there’s no stable address where reminders can reach you?

We understand that life can be unpredictable, that’s why we're building a system, so you are not forgotten.

📌 𝗢𝘂𝘁𝗿𝗲𝗮𝗰𝗵 𝗮𝗻𝗱 𝗙𝗮𝗰𝗲-𝘁𝗼-𝗙𝗮𝗰𝗲 𝗥𝗲𝗺𝗶𝗻𝗱𝗲𝗿𝘀
– Our team works closely with community caseworkers, outreach services, and hospitals to check in and remind clients of their appointments. In some cases, community health nurses and caseworkers can help bring clients to their appointments, ensuring that transportation or logistical barriers never stand in the way of receiving care.

📌 𝗣𝗮𝗿𝘁𝗻𝗲𝗿𝘀𝗵𝗶𝗽𝘀
– We collaborate with homeless relief services who know the community and can help make sure messages get through.

📌 𝗙𝗹𝗲𝘅𝗶𝗯𝗶𝗹𝗶𝘁𝘆 𝗮𝗻𝗱 𝗨𝗻𝗱𝗲𝗿𝘀𝘁𝗮𝗻𝗱𝗶𝗻𝗴
– If someone misses an appointment, we follow up.

Catherine ( in the picture below), from VincentCare, has worked tirelessly, not only referring countless homeless clients to Mobile GP, but also ensuring they don't miss an appointment, even when they’ve moved into housing elsewhere. She’s the one who makes sure they get there. From navigating transportation barriers to reminding them when it’s time to see a doctor, Catherine is the steady hand that guides them, no matter where life takes them.

When was the last time you felt truly listened to by your doctor?

For many, it's not just about treatment, it's about being treated as a human.

Unfortunately, stigma can stand in the way of care, especially for those who are homeless or at risk of homelessness.

Enter Dr. Shish Malgwa, a Telehealth GP from Atticus Health, with over 10 years of experience and a true passion for holistic, patient-centred care.

In the Mobile GP program, he’s not just delivering medical support, he’s offering something far more valuable: 𝗱𝗶𝗴𝗻𝗶𝘁𝘆.

Dr. Shish takes the time to truly listen, offering 30-minute consultations where patients can speak openly, knowing they’re valued.

And the demand is real. Dr. Shish is fully booked tomorrow, and it's a testament to the trust he’s built within the community.

We believe that everyone deserves the opportunity to be heard, and Dr. Shish is helping us make that possible.

Imagine facing every day with uncertainty, not knowing where you’ll sleep tonight or where your next meal will come from.

Now, imagine trying to navigate the healthcare system in that chaos.

For people experiencing homelessness, this is a reality and it’s a reality we can change.

Dr. Gabriel Maia, our Telehealth GP from Atticus Health, is on the frontlines of that change. With nearly 20 years of medical experience, apart from treating people, he’s building relationships and providing a level of stability.

For those at risk of homelessness, continuity of care is EVERYTHING. When you have a trusted GP who knows your history, understands your struggles, and is there for the long haul, you get a partner in health.

When someone is admitted to the Emergency Department, having a regular GP like Dr. Maia means the hospital can send the discharge summary directly to him. The result? Seamless continuity of care. Dr. Maia has access to the latest treatment information, meaning less time wasted on repeating tests or treatments, and faster recovery for the patient.

We’re proud to have Dr. Maia as part of our Mobile GP project.

For many in underserved communities, this is the norm. The lack of continuity of care leads to misdiagnoses, poor health outcomes, and a cycle of crisis-driven treatment instead of long-term wellness.

Continuity of care isn’t just about having the same doctor—it’s about building trust, understanding, and a proactive approach to health. When patients see the same provider over time, their medical history is understood, early interventions become possible, and health management improves.

It was so cool to catch up with Rama today.

Saying we’re inspired is an understatement.

In her 80s, she had one goal: to visit her family in London and explore Europe. But first, she needed to make sure her health wouldn’t hold her back.

Through the Mobile GP program and Mark & Sylvie's Home Care , she got exactly what she needed, a telehealth GP, an onsite nurse for her B12 injections, a face-to-face geriatrician, and a plan tailored to her needs.

The result? Life-changing.

She didn’t just visit her family, she spent two months traveling across 12 European countries. And here’s the incredible part:

She started by walking upto 5km a day. By the end of her journey it was 12-13km daily.

Amazing! 🤩

Rama now understands how to manage her health, giving her the freedom to live on her terms.

Lesson: Yes, healthcare is about treating illness, but it’s also about unlocking possibility. ✨

What does holistic care really mean?

Let’s get personal.

Imagine you’ve been carrying a heavy backpack for years. It’s crammed with life’s weight, mental health struggles, physical pain, and the emotional toll of uncertainty. Now, imagine someone offers to help, but they only pull out one item. Your back still hurts.

This is why holistic care matters. It’s not about putting a Band-Aid on one part of your life. It’s about unpacking the whole bag: your body, your mind, and your heart.

For our Mobile GP patients, this approach can be life-changing. It’s about listening to more than just symptoms, it’s hearing the story behind them. It’s connecting dots between mental health and physical health, between isolation and stability.

If you’re feeling stuck, here’s what you can do today:

1️⃣ Ask for help—without shame. Speak to someone who will see all of you, not just your problem.
2️⃣ Move your body—even a short walk can release tension. Physical health is a gateway to emotional health.
3️⃣ Be kind to yourself—progress takes time, but you’re worth the effort.
