Home Health Rehab are a team of dedicated mobile physiotherapists and occupational therapists providing high quality services to people within their own homes throughout the City of Greater Geelong, Colac, Surf Coast Shire and Bellarine Peninsula.
We are passionate about providing treatment to people in their own homes, as we know first hand the difficultly of attending a clinic for treatment after an injury, illness or with restricted mobility.
About the role
Our growing organisation is seeking a casual or contracting occupational therapist to join our team in the Geelong / Surf Coast / Bellarine region. The suitable applicant will have a strong commitment to customer service, with a high level of interpersonal skills and a desire to work as part of a small tight knit team with a focus on providing the best evidence-based care possible.
This flexible role of 1-2 days per week (with the potential to grow) would suit people looking for some additional work, those who were studying, returning from parental leave or balancing other commitments.
You will have the opportunity to provide person centred care with an opportunity to:
Travel across the Geelong / Surf Coast/ Bellarine region working with community based clients.
Work with clients, carers and families to provide practical solutions to promote independence and mobility at home and in the community
Develop goals and care plans to assist clients to remain at home and active in their local community
Work with a diverse range of clients and health conditions
Conduct comprehensive assessments that identify the clients' functional capacity, strengths and lifestyle goals to develop holistic, customised action plans. In order to do this you will: Conduct home safety assessments, advise on home modifications, equipment to aid ADLs, pressure care, specialised seating/ wheelchair prescription and scooter and mobility assessment and training.
Liaise with case managers / equipment suppliers / builders to help meet the OT needs of the client.

Skills and experience
Essential:
Bachelor of Occupational Therapy degree with full AHPRA registration.
National Police Record Check.
Hold a current motor vehicle license.
Current Level 2 First Aid.
Broad knowledge and experience in the planning, implementation, delivery and evaluation of occupational therapy assessments within community-based settings.
Post graduate experience in a wide range of settings.
Be self motivated with a high level of communication, organizational and time management skills.
Be able to work independently and as a member of a team.
Well developed interpersonal and communication skills.
