SUPPORT AT HOME CARE PARTNER
(Aged Care)

We are currently recruiting a Support at Home Care Partner (Aged Care) to be based in our Horsham office. Hours are up to full time (negotiable)

Care Partners engage in care management and care planning activities to ensure that Support at Home program participants receive the right mix of care, at the right cadence to achieve their set goals (aligned to their care plan). The role ensures culturally appropriate care for participants from diverse backgrounds. The role is responsible for building and maintaining relationships and regularly monitoring, reviewing and evaluating of participant’s changing care needs and goals and ensuring a timely response to changing care needs. Care Partners ensure that participants are supported and educated on their rights and are responsible for ensuring participants receive quality care.
