Across the world, people are living longer than ever before. By 2030, one in six people will be over the age of 60, and by 2050 the number of adults over 80 will triple to 426 million. While longevity is something to celebrate, it also brings challenges. MLD is well know for its benefits in managing...

【two years】

Everyone knows that this year is a big transitional year.
I certainly have been experiencing it.
I knew we had been neglecting each other in this constantly changing time , but I didn’t realised it had been more than two years since our last catch-up!!

I had a nice breakfast catch-up with my cross-occupation yet working-towards-same-purpose therapist friend at a cafe in a cold and drizzly morning🧡

She is far away from judgement.

Therefore, I always feel comfortable with her.

She’s a great interatcial stimulant.

We both have engaged into our study and clinical days for improving chronically affected people’s health by making the lymphatic system working better by our services.

New discoveries and studies in lymphatic system keep springing up.
I have been working in such an exciting industry. More and more people, including medical professions, have slowly been realising about the role of lymphatic system and what MLD (manual lymph drainage) can do for people’s well-being not only reducing excess fluid.

This is my passion along side of Kendo.
Can I keep both at the same time?

I believe so!!!!

We will have another catch-up sooner rather than two year later😆

Most people think of lipedema as a condition that only affects women, but new research is proving otherwise.

A recent study from Brazil followed five men diagnosed with lipedema, all showing the same hallmark signs seen in women: symmetrical fat accumulation in the legs, sparing of the feet, pain, and easy bruising. Many had endocrine disorders, and one tested positive for HLA-DQ2 and HLA-DQ8 - markers linked to gluten sensitivity.

With targeted diet changes, low-impact exercise, and in some cases medication, every patient saw improvements in just a few weeks. This is powerful evidence that lipedema in men is real, underdiagnosed, and treatable.

Flap Breast Reconstruction after mastectomy.

For this type of reconstruction surgery, the surgeon uses the patient’s own tissue from other part of body to rebuild the breast. For this patient, her skin, fat and a portion of the re**us abdominis muscle from the abdomen. Consequently, she has now a large surgical scar.

I’ve been seeing her evey week after her second surgery due to some wound heal complications. The scar is now anout 3 months old, and we extended the treatment every second week.

should keep working helping soften the scared/ fibrotic tissue along the surgical line while I’m not seeing her😊

🔬 Connective Tissue Abnormalities in Lipedema
🧱 Structural Changes
• Thickened fibrotic septa divide fat lobules.
• Dense extracellular matrix (ECM) with excess collagen types I & III.
• Loss of tissue elasticity and reduced skin recoil.
💧 Fluid Retention
• ECM contains excess hyaluronic acid and proteoglycans, which hold water.
• Lymphatic drainage is impaired due to stiffened connective tissue → swelling and heaviness.
🩸 Capillary Fragility
• Fragile capillaries lead to easy bruising with minor pressure or trauma.
🔁 Chronic Inflammation & Tissue Remodeling
• Elevated matrix metalloproteinases (MMP-2, MMP-9) degrade collagen and alter ECM.
• Ongoing fibrosis and low-grade inflammation contribute to disease progression.
💡 Clinical Implications
• Fat is resistant to weight loss through diet or exercise.
• Symptoms include pain, swelling, heaviness, and sensitivity.
• Compression, manual lymphatic drainage (MLD), and liposuction help manage symptoms but do not reverse ECM fibrosis.

🌊 What Is the Glymphatic System?

The glymphatic system is the brain’s unique waste clearance network, functioning similarly to the lymphatic system in the body—but with a twist. It was only discovered in 2012 by Dr. Maiken Nedergaard, and it has since changed how we understand neurodegeneration and brain inflammation.
This system relies on cerebrospinal fluid (CSF) to flush out waste products from brain tissue through perivascular pathways, facilitated by a type of glial cell called astrocytes. These cells regulate the flow of interstitial fluid and act as a conduit for metabolic clearance during deep sleep, especially in slow-wave sleep cycles.

🔥 When the Glymphatic System Is Inhibited: The Inflammatory Storm

When the glymphatic system is impaired, neurotoxic proteins—like beta-amyloid, tau proteins, and inflammatory cytokines—begin to accumulate in the brain's interstitial spaces. This accumulation triggers:
* Microglial activation, leading to chronic low-grade neuroinflammation
* Increased production of pro-inflammatory cytokines like IL-6, TNF-α, and IL-1β
* Oxidative stress and mitochondrial dysfunction within neurons
* Blood-brain barrier permeability ("leaky brain") and further immune dysregulation

Over time, this chronic inflammatory state can manifest as:
* Brain fog, memory issues, and cognitive decline
* Mood disorders such as anxiety and depression
* Increased risk of neurodegenerative conditions like Alzheimer’s and Parkinson’s
* Worsened systemic inflammation due to vagus nerve signaling disruption

🛌 Sleep, the Glymphatic Switch, and Circadian Health

The glymphatic system is most active during deep sleep, particularly during non-REM slow-wave phases. When sleep is disrupted—whether due to stress, screen exposure, sleep apnea, or erratic sleep cycles—the brain cannot engage in glymphatic flushing.
Sleep deprivation has been shown to:
* Increase extracellular beta-amyloid by up to 43% in a single night
* Decrease the expression of aquaporin-4 (AQP4) water channels in astrocytes, impairing fluid transport
* Heighten markers of neuroinflammation, including NF-κB signaling and glial activation

🧬 Systemic Inflammation and Glymphatic Dysfunction: A Two-Way Street

Interestingly, inflammation itself suppresses glymphatic flow. Research shows that systemic infections, autoimmune flares, and even gut dysbiosis can produce pro-inflammatory cytokines that reduce CSF dynamics and glymphatic activity.
Conversely, poor glymphatic clearance can worsen systemic inflammation by:
* Disrupting hypothalamic-pituitary-adrenal (HPA) axis signaling
* Altering vagal tone and the gut-brain-liver immune axis
* Impairing clearance of immune-modulating neurotransmitters like glutamate

🌿 How to Support Glymphatic Health

1. Prioritize Deep Sleep
* Aim for 7–9 hours of uninterrupted sleep in total darkness
* Use magnesium, L-theanine, or glycine to support non-REM sleep
* Avoid screens and caffeine 3+ hours before bedtime
2. Rebound, Stretch, and Move Your Spine
* Movement of the spine and neck enhances CSF circulation
* Manual lymphatic drainage may also indirectly stimulate glymphatic function
3. Hydration & Electrolyte Balance
* CSF production is heavily dependent on fluid status
* Add trace minerals or electrolytes to water to support fluid dynamics
4. Nutraceutical Support
* Resveratrol, turmeric (curcumin), omega-3s, and NAC reduce neuroinflammation
* Melatonin not only promotes deep sleep but enhances glymphatic activity
5. Cranial and Cervical Lymphatic Drainage
* Facial and neck MLD can relieve interstitial congestion
* Techniques like craniosacral therapy or vagal nerve stimulation may further support this network

🧠 Final Thought

The glymphatic system is a vital yet vulnerable detox engine for the brain. When impaired, it doesn’t just affect cognition—it can unleash a cascade of inflammatory dysfunction that spreads throughout the entire body.

By supporting this system through sleep hygiene, lymphatic stimulation, and anti-inflammatory practices, we lay the foundation for resilient mental, neurological, and immune health.

A wonderful way to raise the awareness of Lipoedema.
Go, Dr Lekich.

DR. CHRIS LEKICH
The “Accidental Ultracyclist” Takes on RAAM

Dr. Chris Lekich (Australia) is one of our solo rookies on the roster for this year’s Race Across America (RAAM) recently revealed why he’s “the accidental ultra-cyclist” in an UltraCycling Magazine interview. “I was late to cycling,” he said. “I started about eight years ago and within about 12 months found myself doing the Peaks Challenge Series on the Gold Coast, and then I got talked into doing the Taupo Classic in New Zealand, which is 640 kilometers and 6400 meters of elevation, and I quite enjoyed that.” Dr. Leckich completed the event in about 30 hours, “Then someone tapped me on the shoulder, ‘You've just qualified to do the Race Across America. I very quickly found out about this race, and it's been in my bloodstream and just percolating all that time, and Covid kind of derailed the plans. I've been off the bike. I had a period of about two years off the bike, and in about July of last year, I just started riding again.”

The 58-year-old’s RAAM motivation stems from his commitment to raising awareness for Lipoedema, a misunderstood genetic disease that affects at least 10 to 15 % of women in Australia and worldwide. Lipoedema often starts during puberty where the diseased fat disproportionately weighs down the legs of young girls and women causing pain and psychological distress, worsening with pregnancy and menopause. Learn more about this and Ride to Walk with Freedom campaign at https://lipoedemasurgicalsolution.com/

While he’ll be a RAAM rookie, Lekich will benefit from the vast experience of his crew, which includes 2-time RAAM finisher Alan Jefferson, and Jefferson’s crew chief Greg Huppatz, who’ll also be at the helm of the doctor’s RAAM campaign. Huppatz discussed the crew. “Chris has given me a fair bit of leeway in organizing the crew. Between everyone in the crew, we've got 25 or 26 crossings with Race Across America. So there's a fair bit of experience there. We still have some rookies that will learn all about it in the first 12 hours and then Groundhog Day for the next 10 or 12 days.”

Huppatz also discussed how he’ll tailor things for the doctor, “I don't think I'm speaking out of school when I say sort of Al Jefferson's a bit of a freak to his ability to stay on the bike and ride. So I don't think we can do everything exactly the same for Chris as we've done for Al. But certainly it'll be modified slightly to suit Chris and his riding style. And his needs pretty much will follow the same sort of regime as we have with Al over the last few years.
