It's time to give something back!👊
This year my wonderful team informed me about this great event to raise funds for cancer survivors and research. We have created a team that will collectively be walking for 24 hours straight! At any given moment, atleast one person has to be walking, the same way at any one moment, somebodies life is turned upside down with a diagnosis of Cancer 😔
I personally have loved family members who have passed away from cancer and survived cancer, I have clients turned friends who have been battling cancer so bravely that it breaks my heart and my teams hearts. In reality, we are weak and they are strong, so this is the least we can do to show support.

Hey All😁
Chronic Care Allied Health is looking for their first Occupational Therapist to join our team of Exercise Physiologists! We specialise in neurological rehabilitation and complex disability, but also work with multiple aged care providers other insurance schemes. This is a great opportunity for an Occupational Therapist with a vision to create a leadership role and put in place a culture that will pave the way for future Occupational Therapists that join our team. We are looking for someone with at least 1-2 years experience, as you will be collaborating and with our exercise physiologists on what you see as the best way to complete your role and provide exceptional care to our community of clients.

We are growing something special and the team culture is fun, supportive and collaborative. We want to the best in the rehabilitation sector, which is why we are expanding (within the next couple of months) our brand new clinic to include an upper limb rehab section with robotic and gaming equipment to improve client outcomes. The new area will also have ADL retraining, such as a kitchen area that will be set up with assistive technology and other tools that you can use to help clients practice to be able to live at home independently. We want to expand into a multidisciplinary team, inclusive of Speech Pathologists, Physiotherapists, Dietitians and Psychologists.

If you are fun, respectful, genuinely love making meaningful connecting with your clients and have a passion to keep learning; please reach out because we want you.

Chronic Care Allied Health aims to lead the way in rehabilitation for Neurological and complex disability. Focusing heavily on education, innovation and genuine client connections, this sets us apart from the rest. Our brand new clinic based in Kogarah has been purpose built with specialised and sta...

Hey Everyone,
Thanks to all our clients, refers and friends, we continue to grow and are looking to hire another Full-Time Exercise Physiologist. We are growing something special and the team culture is fun, supportive and collaborative. No one is better than the other and we all learn from each other. We specialise in neurological rehab and complex disability but the case load is mixed and includes workers compensation, cardiac, cancer, musculoskeletal and many other chronic conditions. We want to the best in the rehabilitation sector, which is why we are expanding (within the next couple of months) our brand new clinic to include an upper limb rehab section with robotic and gaming equipment to improve client outcomes. We are also aiming to expand into a multidisciplinary team, currently looking for an Occupational Therapist.

If you are fun, respectful, genuinely love making meaningful connecting with your clients and have a passion to keep learning; please reach out because we want you.

Peter's Story 💪

Peter was diagnosed with Myelin Oligodendrocyte Glycoprotein Antibody Disease (MOGAD) in 2021 following hospitalisation for a virus. MOGAD is an inflammatory disorder of the central nervous system characterised by attacks of immune-mediated demyelination predominantly targeting the optic nerves, brain, and spinal cord. Secondary to his MOGAD diagnosis, Peter was diagnosed with transverse myelitis. Transverse Myelitis is a disorder characterised by inflammation of the spinal cord leading to symptoms of neurological dysfunction including sensory and motor disruptions. Peter was bed bound for around 6 weeks and as a result of his neurological disorder had the mammoth task of relearning how to sit, stand and walk again over the following months.

Peter’s goals centre around improving his functional capacity so he is able to engage in the everyday activities that he loves including cooking and eating great food, walking, golfing and spending time with family and friends.

In sessions, Peter has been focussing on gait retraining, improving his walking capacity, trunk stability and navigating obstacles, steps and stairs. Recently, Peter has been smashing his goals completing a 2.45km walk along the foreshore and heading out to the driving range for the first time since his hospitalisation.

Check out that technique!

This is only the beginning Peter, keep up the amazing work! Watch this space.

Teamwork make the dream work!!!
We had the pleasure of Alvin and Ivana showing us the "Lifelines Neuro" ambulatory EEG recorder for our weekly in-service.
We are always looking for ways to either find or show specific exercise prescriptions impacting the neurophysiology of diseases.

EEG measures the electrical activity of a group of neurons on the most superficial aspect of the brain (cerebral cortex). We had our research hats on and were thinking if there is a way to measure speed of electrical activity in specific cortical areas pre and post treatment, and if this could be an indirect measure of myelin repair in demyelinating diseases. We also measured Amjad's brain activity during overground walking, treadmill walking and on our ICARE robotic gait retrainer to see if there was difference in EEG recordings with each different modality.
In the process, we found that Amjad did not actually have much going on up there anyway🥴

Alvin from Aerobe has opened our eyes to so much mouth watering tools we aim to use for research as well lots of assistive technology at the forefront of neuro rehabilitation. Highly recommend you check out his range and services.

Thank you Eva for being kind enough to give our practice a plug and interview me😁
Just a few corrections that I probably didn't explain properly and are in the article;
- Chronic Care has been operating since 2014, but our brand new clinic has been operating since December.
- We are aiming to treat and improve central nervous system outcomes through exercise, not muscle strength. Although, improved muscle strength is a welcomed benefit that comes with it
- Exercise has been shown to reduce pro inflammatory markers and increase anti inflammatory markers which can be beneficial in MS as it is an autoimmune inflammatory disease.
- Flipping a coin or bouncing a ball, is not going to improve muscle function and walking capacity. I was saying that we have clients with a range of capacities, some may not even be able to get out of bed so we work on bed mobility and rolling, or some might have reduced hand function so we might do exercises such as flipping coins and bouncing balls.
- exercise does improve fatigue in MS, but it's not just as simple as muscles getting fitter. It more so, but not limited to, improving the efficiency of the nervous system being able to stimulate muscles. Hence improving central fatigue.
- Serotonin is an important neurotransmitter and part of the potential reason for improved mental health outcomes with exercise, among many reasons.

Hope that clears some things up! Overall a great article and very much appreciated☺️

Expanding the body's potential.

Patient Stories - Rishi 🏆

Rishi first came to Chronic Care Allied Health following a motorbike accident in 2019 from which he sustained a right side traumatic brain injury in addition to a right brachial plexus injury, broken ribs, punctured lung and surgical emphysema. Rishi had received intensive rehabilitation in Hospital and had made significant gains in his respiratory capacity and strength, was able to walk unaided and lift both arms overhead. Rishi’s goals in his sessions at CCAH focussed on mastering coordination, balance and returning to higher level activities like running, jumping and participating in sports.

Rishi has since returned to running and is enjoying including sports style exercises in his sessions. Recently, Rishi has begun using the stepper at his local gym to help develop his running mechanics.

Way to go Rishi! 💪

How does this exercise help Rishi’s running?
When using the stepper at a high speed Rishi is required to produce rapid hip flexion whilst maintaining trunk stability and absorbing force through his calf. Additionally, Rishi does this exercise with his heels slightly raised to replicate the rapid absorption and generation of plantar flexion forces experienced during running.

⭐️ Chronic Care Champions ⭐️

Danny 🏆

Danny’s first battle with his health began back when he was 3 years old after he was diagnosed with Leukaemia. After successfully beating cancer, Danny unfortunately lost his left lower leg to an infection. In 2016 Danny suffered his first stroke that left him with right side impairments. Over the course of Danny’s rehabilitation journey at Chronic Care Allied Health he has been met with recurrent strokes leading him to be regularly in and out of hospital. After each hospital stay, Danny has had to relearn some of the basic fundamental movement skills that we take for granted like rolling, sitting, standing and walking.

After his last stroke, Danny was unable to sit, stand or walk independently and required a hoist for all transfers. Currently, Danny is able to sit independently, stand and walk short distances with therapist assistance. Danny is striving to return to walking and standing independently so he can head down to his local coffee shop!

Despite the ongoing challenges he continues to face, Danny works extremely hard every session and always manages to put a smile on our faces. Keep up the good work Danny!

How & Why Chronic Care Came to Be

Chronic Care understands that every individual has an innate need for growth and yearning to strive towards their goals. It is therefore our mission to foster this growth under the platform of exercise and lifestyle modification. Chronic Care exists to help our patients achieve their potential thereby improving functional independence and quality of life. We aim to treat the disease at both the impairment level and the secondary complications associated with the disease. The impairment level refers to the actual initial issue such as loss/dysfunction of neuronal connections after a stroke; And the secondary complications are the result of the initial injury such as muscle weakness, increased blood pressure, type 2 diabetes, reduced balance etc. due to reduced physical activity after stroke. We specialise in neurological conditions such as stroke, parkinsons disease and multiple sclerosis, however we have extensive experience in cardiopulmonary and metabolic disease as these are common co morbidities or pre-cursors to neurological disease. As with all things, Chronic Care Exercise Physiology has a beginning, which is the drive behind our mission and just like most stories, this one comes from humble beginnings...

As a teenager through high school I grew up playing rugby league and was very much into strength and conditioning. And as you may know, this comes with its fair share of injuries. From very early on I found myself researching how to rehabilitate my self and from this developed a passion for rehabilitation. Fast Forward a few years and I found my self studying this relatively new degree called Exercise Physiology at the University of NSW. Exercise physiology is the study of the bodies biological response to physical activity and using that information to treat diseases. It is the principle that exercise is MEDICINE, the same way a drug has a specific dosage to be effective so does exercise. We all know how important exercise is but it can be extremely difficult to know which exercises to choose that will help you reach your goals and be safe at the same time. Now imagine you suffer from a chronic disease or disability, that would make starting an exercise program even more daunting and that’s where an Exercise Physiologist comes into the picture.

Four years later I have graduated and out in the real world. I wanted to make a difference and help people achieve independence and improved quality of life. It was 2014 and that was the year Chronic Care Exercise Physiology began. I started off treating family and friends while I was gaining experience working in the public hospital sector. First working at Sutherland Hospital in the Heart Failure and Pulmonary rehab team. Initially most of my experience was in cardiac and metabolic rehab which I enjoyed, however it wasn’t until my time at Prince of Wales Hospital in the rehab ward that I realised my passion was in neurological rehabilitation. Dealing with mainly brain injury and spinal cord injury I could see there was a massive disconnect between the research and clinical practice. The research was screaming the need for aerobic and functional strength conditioning programs for the neurological population but it wasn’t filtering to the public health sector. I then got an interview with a company that was leading the way in neurological rehab working both in the clinic and in the community. Working in a multidisciplinary team of occupational therapists and physiotherapists I gained an invaluable amount of experience and passion for this population group. Which takes us to now...

If you are dealing with any chronic disease or injury and want to find out out more about how we can help you please contact us. We are registered NDIS, Medicare and WorkCover NSW providers. We can provide services at our clinic, in your home or even your local gym.
