Don't rush the return to run process following ACL surgery.

Videos 1 and 3 show what commonly occurs on an athletes first run - a lack of quality knee yield. We need a small but important knee bend on ground contact to help absorb the shock of a landing. If the force isn't absorbed well at the knee and ankle (note the lack of controlled pronation) then it needs to be absorbed at the hip -> resulting in the sudden small hip internal rotation and adduction motion at ground contact - the knee falls inwards.

It's not a disaster, and no harm is likely to come of it but it results in a high (right knee) low (left knee) running pattern that resembles a limp, that's tricky to identify and very hard to train out, if its been missed and ingrained through repetition.

Vids 2 and 4 show significantly better force absorption at the knee. Guess how long this took to change between videos? 1 week. That's all the time Al needed to take the feedback on board and make a positive difference. It helps having an athlete who is a dream to coach. It's another huge help to use slow motion capture, we just cant see enough without it.

The take away message is just because you can run post surgery doesnt always mean you should, waiting a few weeks longer to improve the quality of your run will help save months of corrective work down the track, especially when it comes to sprinting

12 weeks post ACL reconstruction, we see Al having her first attempts at horizontal jumps.

The first video shows a big hip dominant landing strategy. The knee doesnt go forward over the big toe, their isn't much positive shin angle, lots of gripping and guarding with the toes and foot. The force has got to be absorbed somewhere, so its the trunk and hip that bend. Being able to absorb big forces at the hip and trunk is awesome, we just don't want it to be the only strategy an ACL reconstruction develops.

You have to be able to also absorb forces at the knee, especially if like most of us, you were a knee dominant athlete prior to injury.

The second video we practised 'constraint based learning'. Put a fence or a wall in front of the athlete and now they have a subconscious deterent to lean forwards. Perfect. If the athlete is keen on keeping their face the way it was, they'll subconsciously absorb more force at the knee. No coaching outside of 'how did that feel?' And 'why do you think that felt better?'.

Today we had our last session with Mon after nearly four years and three knee injuries later. Itโ€™s fitting that we bring this journey to a close by looking back at when she got cleared to play soccer again after three years of rehab!

Welcome John!!

John Marsden is a qualified physiotherapist with a Master of Physiotherapy and a Bachelor of Exercise and Sports Science, both completed at the University of Tasmania. He has developed valuable clinical experience in musculoskeletal physiotherapy through his involvement in a range of rehabilitation and sports performance settings, working with athletes and individuals experiencing pain during his studies.

With a background in both sport and exercise performance as an athlete and a clinician, John has worked as a sports trainer with Old Scotch, Launceston, and Perth Football Clubs, and has supported AFL teams including Hawthorn, North Melbourne, and the Sydney Swans. These experiences have enhanced his understanding of injury prevention, rehabilitation, and performance optimisation, particularly within the demands of Australian Rules football. He has a strong interest in helping athletes recover from injury and return to their best both physically and mentally.

Johnโ€™s passion for physiotherapy was inspired by his own experience of being sidelined with injury. This personal journey drives his commitment to helping others recover, return to sport, and achieve their goals. He is equally dedicated to supporting individuals in managing pain and improving their quality of life, helping them move better, feel better, and live more active, fulfilling lives.

Wecome Lachie!!!

Lachie grew up in Tasmania before relocating to Melbourne to complete his studies. Lachie completed a Bachelor of Exercise and Sport Science before going on to complete a Doctor of Physiotherapy at the University of Melbourne

Lachie completed his University clinical placements in a diverse range of areas, including private practice, paediatrics, rehabilitation settings, and acute and subacute hospital settings. He has a keen interest on AFL sporting injuries such as hamstring and knee injuries, as well as chronic pain and long term injuries including tendinopathy. Lachie is passionate about providing holistic, athlete-centered rehabilitation, with an emphasis on helping athletes return to their full potential.ย 

Outside of the clinic, Lachie loves playing football. He has previously played for the Launceston Football Club and played for Uni Blues in the VAFA while he was in Melbourne. Lachie plays football for the Old Scotch Collegians Football Club, and is the Head Coach of their Under 18s team. He also supports as one of the Physiotherapists at the club.
