This role offers flexibility, autonomy, and the opportunity to work in a supportive, values-driven environment. You’ll play a key role in overseeing the care and well-being of our clients, working collaboratively with our care support team to deliver exceptional services in clients’ homes.
Key Responsibilities:
🩺 Assess, plan, implement and evaluate person-centred care
👥 Partner with clients, families, and support workers to ensure high-quality outcomes
📋 Complete clinical documentation and maintain compliance standards
🧠 Provide education and support around dementia, medication, and wellness
📞 Respond to clinical escalations and support staff in the field
What we’re looking for:
✅ Current AHPRA registration as a Registered Nurse
✅ Clear communication skills and compassionate care delivery
✅ Clinical experience in community, aged care, or similar setting
✅ Strong time management and problem-solving abilities
✅ Ability to manage time and work autonomously
✅ Police clearance
✅ Current First Aid and CPR
✅ Driver’s licence and roadworthy vehicle
✅ A positive, flexible, and collaborative attitude
✅ Ability to manage time and work autonomously
