Reintroducing cow’s milk back into your baby’s diet when they have cow’s milk protein intolerance (food protein induced allergic proctocolitis):

This can be done as the dairy ladder OR by simply trying fresh cow’s milk. There is no wrong answer here for CMPI. Often parents can feel worried about starting with straight milk. Aside from the worry some kids can still have an intolerance to milk. In these cases they may be able to tolerate something like a milk muffin. For this reason often the dairy ladder is the suggested way of reintroducing cows milk. If confident however, there is no harm in trying cow’s milk first. Always talk to your local doctor regarding the options.

NOTE: the management is different for an IgE mediated allergy to cow’s milk.

Peanut is a legume. Peanut is not a nut. Therefore peanut is not related to other nuts. So most kids with a peanut allergy can eat all other nuts. All other nuts are classified as tree nuts.

Dairy and soy allergy safe supplements

Annoyingly medications do not have food allergens declared on them. Some supplements do contain dairy and are not safe to take with a dairy allergy.

Dairy allergy safe calcium supplement brands in Australia:
Natures way
Ostelin
Swisse
Caltrate
Citracal

Soy in supplements is usually soy lecithin so it is SAFE. Soy lecithin does not cause you to have allergic reactions. This includes anything labelled additive 322 or soy bean oil.

When do I need to remove foods from my diet as a breastfeeding mum?

When breast feeding if your child has the following you need to remove the food/s from your diet:
Cow’s milk protein intolerance (formally termed food protein induced allergic proctocolitis)

You DO NOT need to remove the food from your diet while breast feeding for the following:
-IgE mediated allergies (immediate onset hives, swelling, vomiting, breathing difficulties)
-FPIES (delayed vomiting and/or diarrhoea)
-Eczema

YOU ARE PEANUT ALLERGIC CAN YOU EAT PEANUT OIL?

YES YES YES you can!

You are allergic to the protein part of a food. When the food is processed and made into an oil the protein is removed. So you can definitely usually eat it. EVEN IF your child has had anaphylaxis to the food the oil is safe.

*****EXCEPTIONS
-If the oil has got peanut you can see (cloudy stuff or sediment or the paste) at the bottom of the bottle avoid it. That means it has some residual protein in it, so avoid it!!!
-ALSO if it is labelled ‘cold pressed’, ‘virgin’, ‘high phenolic’, ‘expelled’ or ‘extruded’ it will likely have proteins in it. These oils haven’t been exposed to heat and can retain protein. Avoid these types of oils.

Saying this most of your usual peanut oils from the local supermarket are safe.

If you have a sesame allergy, sesame oil is also generally safe but some people can react to it as it can be less refined. I usually suggest my patients try a small amount to see if they can tolerate it.

CONTACT URTICARIA/HIVES

Non-allergic contact urticaria (hives) is an immediate but transient localised swelling and redness of the skin. It occurs on the skin after it has direct contact with an offending food. The hives are localised to where the food touched the skin and is termed non-immunological i.e. non-allergic.

There are no other symptoms that occur when eating the food. NOTE is hives appear where food HAS NOT touched this skin this is an allergic reaction.

It usually settles on its own within a few hours.

It commonly occurs in children with other atopic conditions like eczema.

How do you manage it?

1. DO NOT avoid the food in your child’s diet. Avoidance can in fact lead to the development of an allergy.
2. The reaction usually improves over time on its own. You usually do not have to do anything at all, just keep feeding them.
3. If the rash is irritating, to avoid the rash occurring you can do the following:
-Wash your child’s skin with a damp wet cloth after the meal.
-You can apply a simple barrier cream before meals to avoid the rash if you wish like a thick greasy moisturising ointment.
-You can give them an antihistamine to reduce the rash if it is causing discomfort.

The most important thing is the keep the food in their diet!

A rarer cause of hives & anaphylaxis

When this young boy would go for a swim he would have anaphylaxis. I diagnosed him with cold induced anaphylaxis after doing the ‘ice cube test’. This is where you place ice on the arm and you develop a welt/hive after removing the ice. He has now been treated for 6months with the medication omalizumab. I have repeated the ice cube test. It was negative, which means he doesn’t develop a hive to the ice anymore. So I let him go for his first swim and he’s done really well, he had no symptoms. I’m allowing to swim again as he wasn’t allowed to anymore 🤗

This is a diagnosis where you have an allergic reaction when you are exposed to cold. Some people with this diagnosis are so sensitive to cold that they get symptoms with colder weather or even when they drink cold water!

⭐️ Don’t be afraid to eat out, you can still do this safely.
⭐️ Avoid eating at a restaurant if the staff seem unsure.

# Call ahead to check the restaurant can cater to your needs. Ask them to speak to the chef directly if the person you are speaking to isn’t sure.
# Check out the menu beforehand online and declare your allergies on the booking.
# ALWAYS declare your allergies again when you are at the restaurant.
⭐️ Even if it’s a regular restaurant you go to always declare any allergies. Why? Things like the ingredients can change.
# Make sure with the waiter that what you are ordering is safe for you to eat. If not ask for options that you can eat. If they are unsure ask them to speak to the chef directly.
# Do not share food utensils e.g. glasses or cutlery

How to safely introduce those allergenic foods at home 🥜 🍳 🥛 🍤 🥣
-Only try 1 new food at a time. If they have an allergic reaction you can be certain about which food was the cause without any confusion.
-You can mix the new food in with something that your child enjoys and tolerates like a vegetable puree. It may make trying the new food a more pleasant experience for your child.
-Start introducing the allergen in the morning rather than the evening, in case they have an allergic reaction.
-If you are nervous about giving your child the new allergen, consider having a support person with you on the day.
-Use an age appropriate form of the food e.g. smooth peanut butter for a baby.

Start with a small amount of the food like a smear. Place it in their mouth, not on their skin. We do not recommend smearing food on the skin as a way of introducing a new food. Every 20-30minutes give your child a bit more of the food, if they are not showing any symptoms of a reaction. Build up to a serve of what you think would be normal for your child to eat.

-IF your child shows any symptoms of an allergic reaction while trying the new food: immediately stop introducing the food and treat their symptoms as needed.

Following this, if they tolerated the food, give them 1 serve of the allergen for 3 consecutive days. If they have no symptoms make sure you then continue it in their diet on a regular basis. This is important as it may protect them against developing an allergy to that food in the future.

Mosquito bites 🦟

A mosquito bite will never cause anaphylaxis. There have never been any cases of mosquito bite anaphylaxis.

Usually a mosquito bite causes local swelling, redness and pain. Sometimes some small blisters can also appear on the skin. The most severe reaction that can occur is what we call a large localised reaction. This is where you get an area of large swelling around the site of the bite. Over years generally your child’s reactions to a mosquito bite improve.

Management (when bitten)
-apply ice as soon as bitten to prevent swelling and pain from occurring (10 minute intervals)
-an antihistamine can be given to reduce symptoms of itch and/or swelling
-panadol or nurofen can be administered to relieve symptoms of pain
-a topical steroid or calamine lotion on the bite can also help relieve symptoms
-avoid scratching to prevent an overlying infection

Prevention (the key is to avoid being bitten)
-use a DEET based insect spray when outdoors
-wear long sleeve clothing where possible
-avoid being outside at dusk, when mosquitoes are out in abundance
-remove any stagnant water from around the household garden where mosquitoes could breed

Egg allergy facts 🥚 🍳

If your child has an egg allergy:
⭐️ They should also avoid eggs from any bird e.g. duck, quail, etc.
⭐️ Your child does not need to avoid chicken meat.
⭐️ You can’t give them the egg yolk, as it is hard to seperate the white from the yolk. You risk your child having an allergic reaction.
⭐️ Egg allergy is more likely to be outgrown than shellfish and nut allergy.
⭐️ They may be able to tolerate baked egg in foods like cake, biscuits or cupcakes. NOTE: Discuss this with your local doctor or allergy specialist before trying these foods in your child’s diet. Some children with an egg allergy can still react to baked goods.
