In Touch support Services Currently has some capacity for our Registered nurse with over 30 years of experience to create care plans for participants that manage their own staff. We also have capacity for continent assessments.
The Care plan will clearly outline the participant’s daily care needs, routines, preferences, and goals.
Ensures that support workers (especially those hired directly by the participant) have a consistent and professional reference to follow.
1. Promotes Safety and Quality
Includes clinical instructions (e.g., medication management, mobility support, nutrition, wound care) to ensure safe and effective care.
Helps reduce risks by identifying early warning signs, emergency procedures, and escalation pathways.
2. Supports NDIS Funding and Compliance
Demonstrates the clinical justification for supports, which can help secure or maintain NDIS funding.
Aligns with NDIS documentation standards, making it easier for participants to navigate reviews and audits.
3. Empowers Participants
Reflects the participant’s voice, choices, and goals, promoting autonomy and dignity.
Helps participants manage their own team with confidence, knowing their care is structured and well-documented.
4. Facilitates Team Communication
Acts as a central reference for all involved: support workers, coordinators, allied health professionals, and family.
Reduces misunderstandings and improves coordination across the participant’s care network.
