Feline auto immune disease 🤔

Is this less common than canine auto immune disease? Or, are we mistaking it for allergies? The truth is, the lesions can look quite similar 🤯

Some tips to help you decide whether a cat could have more going on than just ‘allergies’;
🐱 Lesions do not respond, or only partially respond to anti-pruritics / anti - inflammatory medications
🐱 There is weight loss despite good nutrition and a normal appetite
🐱 The cat is non-pruritic
🐱 The mucocutaneous junctions, claw folds and or extremities are involved

There are exceptions to the above of course, but if in doubt, refer or reach out to your local veterinary dermatologist for advice 🧑‍⚕️

CO2 laser is such a versatile treatment! 🤩

Here, Dr Callum is using the laser to remove a mass growing in a patient’s external ear canal opening.

Masses in these locations inhibit the ear’s normal self-cleaning mechanism, leading to cerumen (wax) build up and secondary infections 🦠

CO2 laser is preferable to conventional surgery in this instance as it allows for:

🌟 Reduced bleeding = less ‘ooze’ post operatively.
🌟 Faster procedure time = shorter anaesthetic.
🌟 More precise excision = less collateral damage to surrounding tissue & less bruising

We’ll be posting more laser surgeries over the coming months to highlight the vast uses of this wonderful treatment modality we have at VDC.

Callum will be discussing all aspects of canine atopy; including current and future anti-pruritics as well as allergy testing and immunotherapy in dogs (allergen desensitisation) 💉🐶

Callum is a board-certified veterinary dermatologist who completed his residency training in Colorado, USA 🇺🇸 before returning home to his roots - Sydney’s inner west 🍸 Callum is passionate about providing the very best standard of care to each and every one of his patients. Co2 laser 💥

Laser surgery is redefining how we practice in veterinary dermatology. VDC have our very own laser located at our clinic in Annandale 📍

Faster healing time, less bleeding, reduced damage & inflammation to surrounding tissues, minimised risk of post operative infection are just some of the benefits of Co2 laser 👏

Laser is now preferred to traditional surgical approaches for many dermatological procedures.

Co2 laser uses:

* Mass removal and/or biopsies
* Sebaceous adenoma, hyperplasia
* Ear canal polyps
* Ablating (resurfacing) the skin surface
* Actinic (solar) dermatitis and skin cancers
* Canine viral papilloma (‘warts’)
* Feline bowenoid in situ carcinoma
* Removal of abnormal tissue, lumps etc

Here, Dr Taryn & Dr Callum are removing an unusual mass from the pinna (ear flap) of a dog. The mass is then sent to an external laboratory for histopathology for diagnostic purposes 🔬👩🏼‍🔬

There is minimal bleeding or trauma involved, improving patient outcome! 🙌

Otitis in dogs👂🐶 🐾

Val 💜 is a beautiful Hungarian vizsla who was referred to VDC for chronic, unilateral Malassezia (yeast) otitis. Unfortunately, Val’s ear infection was refractory to most treatments trialled.

A CT scan was recommended to investigate for potential otitis media. Ear disease can progress to involve the middle ear in cases where the ear drum is perforated. A CT scan is the only image modality that can accurately assess the tympanic bulla and surrounding structures for pathology 🩻

A veterinary radiologist is consulted to review each patients CT scan, and in Val’s case, her CT ruled out otitis media 🧫

Next, Val underwent a deep ear flush using video otoendoscopy to thoroughly cleanse the external ear canal and remove cerumen 🧴 (wax, debris and other accumulations). This is extremely helpful in clearing chronic ear infections.

Val’s yeast infection has now resolved, after a thorough diagnostic work up, successful ear flush and careful treatment plan 📝 We are soooo pleased to have a happy & comfortable Val back! 💜

A brief glimpse into our very first VDC + TPS collab for our Winter derm CE night

Dr Alex, Board Certified Veterinary Dermatologist 👩🏼‍⚕️ presented on atopic dermatitis, ☘️🌸 explaining the complex nature of this disease as well as the process & benefits of allergy testing and desensitisation in pets 💉 Updates on immunotherapy in human & veterinary medicine were also discussed 📚

Billy, the bravest boy 🌟

After many months of failed medications, Billy was diagnosed with both Facial discoid lupus erythematosus (FDLE) and Ischaemic dermatopathy.

These are auto immune diseases that present quite differently. It is not common for a patient to have both conditions concurrently!

FDLE is one of four cutaneous lupoid diseases that primarily causes depigmentation, ulceration and erythema to the nasal planum, nasal bridge, muzzle, face and pinnae.

Ischaemic dermatopathy is a type of cutaneous vasculopathy that falls under the ‘cell poor’ category. Lesions include alopecia, erythema, scale and scarring to the extremities (pinnae, paws, tail tip etc.) These occur due to altered blood flow and small vessel inflammation.

Both dermatological conditions require histopathology (skin biopsy) in order to diagnose & prescribe the correct oral and topical treatments.

Billy is taking a combination of immunomodulatory medications, anti-inflammatories & using various topical products to repair his defective skin barrier function.

Ongoing nutritional support (high protein diet, oral essential fatty acids etc) and UV protection such as mineral sunscreen (to prevent flares of immune mediated diseases) are essential for Billy’s ongoing skin health.

We love Billy and are so proud of his immense progress 💕

Auricular chondritis 👂

What’s that?
This is an immune mediated condition that causes pronounced swelling of the pinnae (ear flaps) 🐶🐱The ears become erythemic (red), inflamed and thickened. This is also a very painful condition and so a quick diagnoses is key 👩🏼‍⚕️💉

What’s the treatment?
Anti inflammatories, such as steroids 💊are usually prescribed for humans with auricular chondritis. Dogs and cats also respond well to prednisolone.

What’s the outcome?
Like any immune driven process, auricular chondritis can wax and wane, so it’s important to continue treatment until clinical signs have resolved. Ongoing monitoring of the pinnae is also required.

Mochi presented to the VDC team who performed biopsies to confirm the diagnosis. We are so pleased to see her progress after 4 weeks of treatment. Mochi’s owner says she is back to her normal happy self 😊
