Advance Care Planning isn’t about paperwork, it’s about people.

It’s about making sure the people who care about you understand what matters most to you, especially if there comes a time when you can’t speak for yourself. These conversations can feel big, but they often bring a surprising sense of relief, clarity, and connection.

Many families say they wish they had talked earlier. One story shared through Advance Care Planning Australia tells of Marie, who sat down at her kitchen table with her son Nico to talk about her wishes. It wasn’t dramatic or rushed, just a thoughtful conversation about what quality of life meant to her. Later, Nico said that knowing his mum’s wishes gave him comfort and confidence during a difficult time. He didn’t have to guess. He knew.

That’s the heart of advance care planning: removing uncertainty for the people you love and making sure your voice is still heard.

It doesn’t have to be done all at once. You can take your time, talk in stages, and revisit your plans as life changes.

You remain in control.
