You can pick up a free Advance Care Directive (ACD) Kit in locations across South Australia.

There are now 123 collection points across 48 council areas, including MP offices, some libraries, Service SA customer service centres, and other sites. To find one near you, visit brnw.ch/21wVRA2

The ACD Kit has been updated to make it easier for people to complete an ACD themselves, without help from a professional.

You can also complete the ACD form or download it from the website advancecaredirectives.sa.gov.au
Existing completed ACD forms remain legally valid, with no requirement to update them using the new form.

❤️❤️❤️ It's World Heart Day, and what better day for us to announce that our team is now delivering the Enhanced Integrated Primary Care (eIPC) model in the Murray Mallee region — a step forward in delivering comprehensive, culturally safe, and condition-specific care for our communities.

This service will be delivered from the new Mannum Medical Centre with a strong focus on chronic disease prevention and management, particularly cardiovascular conditions. Speak to your doctor for a referral to our program if you would like to improve your heart health.

Through multidisciplinary teams and collaboration with small or solo general practices, our dedicated cardiac nurse will help deliver holistic care to underserved populations, including:

✅ Aboriginal and Torres Strait Islander peoples
✅ People experiencing financial disadvantage
✅ Individuals at risk of or living with chronic illness

The eIPC model prioritises health assessments, education, and wraparound support to empower patients and improve long-term outcomes.
