Are you teaching knee drops!? There is insufficient evidence to support its use, in fact it’s likely to potentially worsen coexisting injuries • Stop with the knee drops.

Cross et al. showed that, in blast injuries, 10% of unilateral amputations, 30% of bilateral amputations, and 39% of bilateral transfemoral amputations had concurrent pelvic fractures.

From Penn-Barwell et al., of bilateral lower limb amputations 14% of patients sustained an open pelvic fracture. Perineal/genital injury was a feature in 19 (44%) patients, ranging from unilateral orchidectomy to loss of genitalia and permanent requirement for colostomy and urostomy.

Benfiled et al. showed of 1234 trauma admissions of which 32 presented with multiple extremity amputations (2.5%). There were 13 bilateral amputations and nine triple amputees (bilateral amputations plus upper extremity). The admission amputation levels were as follows: 29 below-the-knee amputations (BKAs), 12 above-the-knee amputations (AKAs), three through-knee amputations (TKAs), one above-elbow (AEA) and eight below-elbow amputations (BEAs). Pelvic fractures and/or ring injuries occurred in 12 patients (55%), of which 46% were considered open and 54% unstable. Pelvic floor and/or perineal blast injuries were not uncommon (16 of 22 patients, 73%).
