What is a forehead flap in nasal reconstruction?

A forehead flap is a time-tested surgical technique used to reconstruct parts of the nose, often after the removal of skin cancers or traumatic injury. It involves using skin and soft tissue from the forehead - which closely matches the nose in texture and colour - to repair or rebuild areas where tissue is missing.

The procedure is typically done in stages:

Stage 1 – Tissue is lifted from the forehead and transferred to the nose, remaining connected to its blood supply.
Stage 2 – The flap may be thinned and shaped, and cartilage can be added if needed for support.
Stage 3 – The flap is divided from the forehead and carefully positioned into its final form.

This approach has been used for centuries and remains one of the most reliable methods for complex nasal reconstruction today.

Carrying forward a centuries-old legacy of nasal reconstruction. In a recent post, we shared historical illustrations from the 1800s that showed early surgical techniques for nasal reconstruction - practices with roots in ancient India and further developed in Renaissance Italy.

This post shows clinical images before and 3 months after a three-stage nasal reconstruction with a Folded Forehead (paramedian) Flap, performed following the removal of a skin cancer.

The staged procedure involved:

Stage 1: Templating the nasal defect up to the forehead, elevating it as flap on a skin bridge (pedicle) connected to the eyebrow and rotating it down to fill the nasal space left by the skin cancer. The forehead is closed and a full thickness skin graft is sutured to the back of the skin bridge.

Stage 2: Elevating and thinning the flap and rebuilding the underlying nasal cartilages for support and definition. The flap is then sutured back into the nasal defect.

Stage 3: Completing the reconstruction by dividing and removing the pedicle (skin bridge) between the nose and forehead.

Each stage is performed under general anaesthesia two weeks between each stage to ensure blood supply is established to the tissues.

This method follows long-established reconstructive principles, adapted to each individual’s needs using modern surgical techniques.

Plastic surgery takes its origins from Sushruta, an Indian surgeon practicing 2500 years ago, and Gaspare Tagliacozzi, who practiced in Italy 500 years ago.

This striking illustration from an 1848 surgical text, Précis iconographique de médecine opératoire et d’anatomie chirurgicale by Chaude Bernard and Charles Huette, depicts early techniques with skin flaps to reconstruct partial or total defects of the nose.

Today, as a Specialist Plastic Surgeon, Dr Jeremy Richardson carries forward this legacy of nasal reconstruction in applying advanced, evidence-based techniques and reconstructive principles to patient care.
