What a month. Many wins, each with its own significance. Some participants joined our activity groups for the first time and met new people. There was a new job for one participant, and even a proposal. We’re grateful to see participants taking steps and connecting. 🙌🏼

When care is consistent, so much more can become possible:

✨ Connection reminds us we’re not alone.
✨ Safety quiets fear.
✨ Confidence grows as encouragement fuels progress.
✨ Growth flourishes when stability gives courage to dream bigger.

At Choice Community Health, we know small acts of presence don’t just help in the moment, they shape a brighter future.

Let’s keep choosing connection, courage, and care, together. 💛

When life gets messy, sometimes all someone needs is to know they won’t be left alone. And for Jess, that made all the difference.

Jessica shared that what helped her most was having someone calm and practical, and who would stick with her. Her support worker Emma stayed through those difficult hours, giving her the confidence to take the next steps toward safety and healing.

These are the everyday moments of care that shape the heart of our work at Choice.

If you or someone you love needs support, we are here.

Choose Choice. Choose Community. Choose Health.

R U OK? Day on 11 September is an important reminder. But at Choice Community Health, we believe checking in on someone's mental health should be part of everyday life.

Many people carry silent struggles. A caring question can offer a moment of connection and help someone find the courage to speak up. You don't need to be a counsellor. You just need to show up and listen.

We’ve seen how something simple, like a genuine check-in, can make a real difference. A colleague who seemed distant. A friend who stopped returning calls. One small question can change the direction of someone’s day.

This R U OK? Day, and every day, let’s build a culture where mental health matters.

Rain or shine, our community comes together 💛 Today’s BBQ was filled with footy colours, shared meals, and plenty of laughter. Moments like these remind us that belonging and connection make life richer, stronger, and full of hope.

Have you ever wanted to join in but found it harder than it should be?

We work alongside people to make participation possible. Helping with transport, finding accessible activities, and providing the right support to feel comfortable and included.

When those barriers are reduced, connection and opportunity become much easier to reach.

✨ Love, surprise & community ✨

Mark and Sarah’s love story has been five years in the making. Earlier this year, with the support of her Choice workers, Sarah returned from Adelaide and surprised Mark at Dandenong Plaza. His face said it all — he was ready.

With the help of friends and support staff, Mark planned something unforgettable. What looked like a makeover photoshoot turned into the surprise of a lifetime.

On Sarah’s 40th birthday, at and surrounded by loved ones, he got down on one knee… She said YES! 💍🎉

It was a day full of love, laughter, and community — a memory they’ll carry forever. 💜

Constant changes to your NDIS plan can make it hard to feel secure and plan ahead.

We help make sense of the changes so you can stay focused on what matters.

4 in 5 NDIS participants have had at least one change to their plan in the past year. These can include pricing updates, shorter plan durations, or adjustments to funding categories.

With the right support, changes don’t have to disrupt your goals or your confidence.

The NDIS continues to evolve, and for many participants and families, that’s brought more questions than answers.

From new pricing rules to shorter plans and delayed community supports, 2025 has already introduced significant changes. If things feel harder to navigate, you’re not alone.

We’ve broken down what’s changing and how we’re supporting people to move through it with clarity and confidence.

Feeling hopeful can be hard when the future feels so uncertain. We focus on making it clearer.

Psychologist C.R. Snyder’s research shows that hope grows when three things come together: knowing what you want, seeing the ways to get there, and believing you can take those steps.

Some people don’t just do their job — they fight with everything they have to make sure someone is seen, heard, and respected.

For the past two years, Deirdre from Disability Justice Australia has been a relentless, compassionate force in Mark’s life — making sure his voice was heard and his rights upheld.

Thanks to her advocacy, Mark is now able to stay in Melbourne — where he feels safe, supported, and empowered to step into the fullness of life after years of being silenced.

It’s been a long road, and Liz from our team has walked it closely with Deirdre every step of the way. But it was Deirdre’s fierce commitment to justice that helped shift the system and protect Mark’s right to choose.

To Deirdre — and the rest of the Disability Justice Australia team who fight tirelessly behind the scenes — thank you. Your work is life-changing.
