At SACC, we are proud to have delivered the South East Care Pathway (SECP) for more than three years. SECP supports people with increased vulnerability and/or complex health needs following hospital discharge, as well as community-based patients at higher risk of hospital admission who require additional support.
South East Care Pathway is a free, nurse-led telehealth service that extends clinical care and helps reduce avoidable emergency department presentations and hospital admissions.
The program is delivered by Sandringham Ambulatory Care Centre (SACC) across south east Melbourne and is funded by South Eastern Melbourne Primary Health Network (SEMPHN).
