CENA Pulse Point - Positioning in bariatric patients with dyspnoea

In the emergency department, we frequently encounter bariatric patients who present with shortness of breath. In one such case, a patient was placed upright at 90 degrees in bed - but still remained very short of breath.

The key insight? For many bariatric patients, a semi‑reclined position of approximately 45 to 60 degrees can significantly enhance comfort and respiratory function. In this posture the abdominal pannus is less likely to push the diaphragm upwards, allowing the diaphragm to move down and the lungs to expand more effectively.
