Your Community Health (previously known as Darebin Community Health) is a health and wellbeing service provider. We offer free and affordable services to anyone in northern Melbourne. We provide services at our three health centres:

East Reservoir Health Centre, 125 Blake Street, East Reservoir, VIC 3073

Northcote Health Centre, 42 Separation Street, Northcote, VIC 3070

PANCH (Preston) Health Centre, 300 Bell Street, Preston, VIC 3072

We also provide some services are available in your home or in other community settings, such as Reservoir Leisure Centre. Our services include:

This April and May, communities across the country will come together for the Walk for Truth. This is a powerful national journey calling for truth-telling, healing and meaningful change for First Nations peoples.

Led by Kerrupmara Gunditjmara man Travis Lovett, the walk begins in Naarm (Melbourne) on 19 April and will travel more than 800km to Canberra, encouraging Australians to walk together and support a national truth-telling process.

Your Community Health will be walking the Preston to Wollert leg, joining others in our community to listen, learn and stand alongside First Nations voices.

This week is about building awareness of lymphoedema, a chronic condition caused by a malfunction of the lymphatic system. It can affect anyone, at any age, and often has a significant impact on physical comfort, daily life and overall wellbeing.

There is no cure, but early recognition and the right support can help manage symptoms and prevent the condition from progressing.

Our ECA team supports children under 9 years of age with Early Childhood Supports on their NDIS plan.

New service highlights include:
- Speech Pathology & Occupational Therapy
