Planning Great Futures Planning Great Futures is a planning consultancy for individuals with a disability, their families, teams and agencies creating exciting futures.

Planning Great Futures has been in business for thirteen years and has completed a number of projects and over seventy graphic plans ( PATH)
Planning - We have done a number of graphic plans PATH and MAP for individuals with a disability, carers, teams and agencies
Carers - We work with carers to create plans for them and help make them happen
Workshops on planning - we deliver fun 'hands on'

workshops for workers on planning including one day workshop 101 ways to plan and a longer program The Tool Kit. these are often followed up by sessions in the workplace. The Housing Roadshow - a workshop for individuals with a disability and their families to provide an informative practical guide to housing and the road to independent living. Includes the booklet The Housing Information Kit. Can also be delivered to workers
Mentor and support program - we offer a program of skill development, links to community, working alongside staff in the workplace to change values, build skills and create a change of culture in services. This can be particularly helpful where traditional training programs have not led to positive change. Participant reviews - a fun filled program including arts, one to one and group work to find out what participants think about their program, and what they would like to change. Suits traditional services wanting to make genuine changes to the way they deliver support.

Hi Carers, the following groups and workshops came out of a Carers Connecting grant from Carers Victoria. Open to all carers and ex carers. And of course Wavlink is also a community house for individuals with a disability.

New Carer Group
We are planning a monthly carers group which is free and open to all carers
First Thursday of each month
Thursday 4th September 1.30 to 3.30 pm
Wheelers Hill Library meeting room, 860, Ferntree Gully Road, Wheelers Hill
A warm welcome, delicious afternoon tea and your ideas about what you would like from the group

Workshop - Planning for the Future
Friday 19th September 1.30 pm to 3.30 pm
An information session at Wavlink Community house for parents and families on how to begin to plan for the future including respite care options, independent living skills, housing choices and who can help with legal matters.
Follow up support for individual questions will be offered.
Afternoon tea included - Speaker Di Gow

Weekly free classes for carers
We have some funding for carer classes on Friday afternoons. Do let us know what you
might enjoy. We can offer IT , art and craft, card making, ceramics and textiles. We can discuss at the carer group meeting . Held at Wavlink Community House

School holidays are just around the corner. Brookfield will be running only one art workshop in the Sept/October holidays. NDIS rules preclude us from charging for the art teacher, materials lunch etc. So Tuesday 23rd September will see Allison our art teacher back and lots of fun activities, delicious lunch and lots of animals to meet and feed.

Dear Support Coordinators, participants and familly carers,
In the past three months we have accepted requests for an increasing number of weekend bookings. We have always charged weekday rates for weekend respite but rising costs including wages, food, rent and activities have increased.

In the future we will charge weekend STA rates
The rates are listed below
STA and assistance including respite care
1:1 Saturday - $2785.13
1:1 Sunday - $3527.69
1:2 Saturday - $1501.97
1;2 Sunday - $1873.25

In cases where we charge under other line items such as Community Participation we will use the weekend rates.
We will always provide a quote for approval before we book weekend respite care to ensure there are sufficient funds and an agreement to pay the appropriate rates.

Brookfield is often asked about weekend stays. We now have some new staff who are young and energetic who are keen to support participants to enjoy weekends at Brookfield farm
We support NDIS participants over ten years, we already have several teenagers who are in foster care or living with their families.

We know some people with disabilites do not always have NDIS funding. Sometime they have applied and are waiting for approval, some participants have gaps when they lose funding for some time or they may not have qualified
Wavlink is a community house in Glen Waverley that offers classes six days a week to students with disabilities.
We have HACC Home and Community care funding that can support individuals without NDIS funding
So if you know someone who is not currently receiving NDIS fundins but has a disability, chronic illness, mental health challenges do contact me on 0412 535 435 about how we can support you.,
HACC funding also covers family carers who may wish to join classes.
