A big congratulations to one of our Surgeons Mr Iman Bayat on his successful mission with Global Vascular Companionship to Ethiopia for the Vascular Surgery Workshop from the 8th to 18th of January.

During the workshop education was provided for local surgeons aspiring to become Vascular surgeons by seeing patients in the outpatients clinic, hands on training during vascular operations and clinic based procedures.

Yarra Vascular Imaging is proud to have supported the mission with donation of funds and also equipment for ultrasound guided sclerotherapy, which had not previously been performed in Jimma, Ethiopia.

CEAP STAGE 5/6

Venous ulcers, also known as venous stasis ulcers, are chronic wounds that develop when veins fail to efficiently return blood from the legs to the heart, causing blood to pool in the lower extremities.

These ulcers typically form around the ankles or lower calves and are characterised by shallow and often irregularly shaped open sores. They are a common complication of chronic venous insufficiency, often seen in individuals with a history of varicose veins or deep vein thrombosis.

Venous ulcers are notorious for their slow healing process and tendency to recur if not properly managed.

Treatment involves addressing the underlying venous insufficiency (varicose veins), managing wound care through regular cleaning and dressings, and often using compression therapy to improve circulation and aid in healing.

CEAP STAGE 4 - Skin changes

Varicose veins can lead to several noticeable changes in the skin. One common skin change associated with varicose veins is brown pigmentation.

The skin around the veins may appear brown, dry and itchy (with flaky skin), or sometimes red and swollen (phlebitis or dermatitis). These skin changes can can lead to the development of skin ulcers or sores. Venous related ulcers can be slow to heal and prone to infection.

It is recommended that individuals with varicose veins causing skin changes seek medical attention and referral to a vascular surgeon, who can advise on appropriate treatment to prevent further skin complications.

Compression stockings are an effective treatment for individuals in the management of chronic venous insufficiency and associated conditions such as swelling (CEAP stage 3). Compression stockings provide graduated pressure on the legs, with the highest pressure at the ankle and gradually decreasing pressure as they move up the leg. This compression helps to improve venous return by assisting the calf muscles in pumping blood back to the heart.

Some of the key benefits of wearing compression stockings include: improved blood circulation, reduction in swelling, relief of discomfort and heaviness, providing support, as well as preventing and aiding in ulcer healing.

It is important to consult with your practitioner to ensure the right compression level and style is recommended on your specific condition. They can also provide personalised recommendations to ensure a proper fit to maximise the benefits of compression therapy for venous swelling.

CEAP stage 3

Represents swelling due to venous disease that is characterised by the presence of oedema, or swelling, in the lower limb due to chronic venous insufficiency.
This results from either valve incompetence or obstruction in the veins, which leads to increased hydrostatic pressure and venous hypertension in the lower extremities.

Swelling can cause discomfort, aches and pains, and later deteriorate to skin changes such as discolouration, thickening and/or hardening and ulceration.

Treatment options for CEAP stage 3 starts with a thorough venous duplex ultrasound scan to assess insufficiency and also deep venous obstruction and in some cases, surgery may be necessary. Compression stockings can alleviate some of the symptoms as can exercising regularly and maintaining a healthy weight.

WHAT IS SCLEROTHERAPY?


Sclerotherapy is a minimally invasive medical procedure used to treat varicose and spider veins. It involves the injection of a chemical solution (sclerosant) into the affected veins, which causes them to collapse and become sealed shut. The body then naturally re-directs blood flow through healthier veins. Depending on the location of your veins, this may need to be done with ultrasound guidance. The procedure usually takes less than an hour and does not require anaesthesia. Patients can resume normal activities immediately after the procedure. In most instances, it may take a few weeks to notice the full results, and multiple treatments may be necessary for complete treatment.

Spider veins are small, damaged veins that appear as red or blue lines visible beneath the surface of the skin.
They often form in clusters, resembling spider webs or tree branches. They are usually not painful, but patient’s may have discomfort or aching if associated with other underlying venous disease.
Spider veins are caused by weakened or damaged valves in the veins, which allow blood to pool and the veins to become enlarged and visible.
Treatment includes microsclerotherapy, compression stockings, lifestyle changes, laser therapy, and minimal invasive vein surgery.

CEAP is a medical classification system that is used to determine the severity of chronic venous disease. CEAP is an acronym for the different categories that describe the different stages of venous disease; Clinical, Etiologic, Anatomic and Pathophysiologic

The CEAP classification system has seven stages, ranging from 0 to 6, with each stage indicating a higher degree of severity.

These stages include no visible signs of venous disease (C0), spider veins (C1), varicose veins (C2), chronic oedema(C3), skin changes(C4), and active or healed venous ulcers (C5 and C6).
