Good intentions are not evidence.
Under Standard 1 of the Strengthened Aged Care Quality Standards, care plans must document the person — their life history, personal goals, cultural identity, communication preferences, and dignity of risk conversations.

A schedule of care tasks does not meet this threshold.
In the file reviews I conduct, care is often being delivered well. The evidence of compliance is absent.

For residents and consumers from CALD backgrounds, this gap is even more consistent — cultural identity, language needs, and spiritual beliefs undocumented in files that are otherwise considered complete.
Undocumented care is non-compliant care.

When did your organisation last complete a structured self-assessment against the Strengthened Aged Care Quality Standards?
Under Standard 2, self-assessment is not a pre-audit activity. It is an ongoing quality obligation — expected by the ACQSC as an embedded process, not a document completed for registration and filed.
Most providers don't have a structured self-assessment framework in place. That is a current non-compliance under the Aged Care Act 2024.

When did your governing body last receive clinical quality data?
Under Standard 5 of the Strengthened Aged Care Quality Standards, governing bodies must actively oversee clinical safety and quality — documented, evidenced, and reported at every meeting.

In the facility reviews I conduct: 12 months of meeting minutes. Zero references to falls rates, pressure injury prevalence, medication incidents, or SIRS notifications.
That is non-compliance with Standard 5 — and a personal accountability for every governing body member under the Aged Care Act 2024.

If your board isn't receiving structured clinical quality reports, that needs to change before your next ACQSC review.

Most aged care providers think they're compliant — until the Commission reviews their evidence.

Across readiness reviews for existing providers, the same issues appear repeatedly:

• Care plans that are task-focused rather than person-centred
• Clinical risks identified but not formally assessed using validated tools
• Incident management processes not consistently applied
• Limited documented evidence of governing body oversight

These are exactly the areas ACQSC reviewers examine during provider re-registration.

Many providers only discover these issues after receiving a Request for Information (RFI) from the Commission — at which point the pressure is significant.
