🚨 Major Changes for Home Care Providers: Are You Ready for November 2025? 🚨

The Department of Health has released the Provider Operational Readiness Priority Actions List—and it’s a wake-up call for Home Care Providers transitioning to the Support at Home program and new Aged Care Act.

💡 Key Actions You Should Be Taking NOW:

✅ Engage with your current HCP clients — new service agreements and care plan discussions should already be underway.
✅ Ensure all client information is updated in My Aged Care and they’re properly assessed.
✅ Check your GPMS and Aged Care Gateway Portal registration info is accurate — this impacts your ability to operate under the new categories.
✅ Align your documents and systems to reflect the Statement of Rights and Principles.
✅ Update and train your workforce on trauma-informed care, co-design, and new digital systems.
✅ Start preparing your financial reports under the new model and review your Support at Home pricing.

📌 This is not just a policy update — it’s a full system shift.
From July, you’ll be expected to start showing compliance, with full enforcement from November 2025.
