This role is ideal for someone with a background in case management, social work, intake coordination, community services, or health care navigation who is passionate about supporting patients and families at critical transition points.

As the Hospital Liaison Manager, you will play a key role in supporting hospital teams and patients to plan safe, timely discharges. You’ll work closely with hospital clinicians and Prestige’s care coordination teams to identify appropriate supports, coordinate services, and ensure continuity of care beyond discharge.

This is a relationship‑focused, client‑centred role, prioritising advocacy, system navigation, and coordination.

This position is a 12-month maternity leave contract, available for a commencement at short notice.

It is a day to remember the bravery, courage and sacrifice made by those who fought for their countries. We also pay our respects to our veteran clients; it is a privilege to support them with care, and we thank them for their service.

The Australian Government has recently announced that older Australians receiving ‘essential personal care’ under the Support at Home Program will no longer face out-of-pocket costs as early as Oct 1st 2026, as these services will be fully government funded. This is a significant win - one that we have been actively advocating for, alongside consumer peak bodies.

The Hon Sam Rae, Minister for Aged Care and Seniors says 'Older Australians, their families and providers told us these services needed to be protected. We've listened, and we're acting.'

Our client Rosslyn was in disbelief when she was told she had won the 20 year anniversary giveaway, saying “it had brightened her world.”

Rosslyn had the opportunity to go to Melbourne Zoo last month with Case Manager Hayley and carer Shirley.

One of the highlights of the day was the visit to the Giraffe Cafe and making new giraffe friends including one she got to take home!
