Knee osteoarthritis Clinical Care Standards: revised

More than 53,000 knee replacements are performed each year in Australia. Yet many people with knee osteoarthritis can manage effectively without surgery.

Australian Commission on Safety and Quality in Health Care has updated the Osteoarthritis of the Knee to guide best practice care.

Well worth a listen! A great discussion on how complex pain is and how simple/mechanical treatments like surgery and many types of physical treatments for many common musculoskeletal conditions are not actually helping - and in some cases causing a lot of harm.

I get a lot of people come in asking me to assess them, wanting to know if their back is "out", hoping I can just put it back "in" thinking this with fix/remove their pain. Similar to the discussion here, my response is usually something like "it doesn't really work like that" and that it is a very reductionist way of thinking of a extremely complex thing. However, the treatment doesn't have to be complex! Once you can be assured that there is nothing serious going on, simple things like healthy lifestyle, education, movement and time to build confidence again can really help.

In this episode of The Shoulder Physio Podcast, I am joined by Rachelle Buchbinder and Ian Harris. Rachelle and Ian are academic doctors who still see and treat patients whilst also conducting high quality research. This conversation centres around the concept that modern medical care is sick....

Always the first thing I educate patients who come in with lateral hip pain (gluteal tendinopathy/bursitis)!
Activity modification makes a huge difference, add a graded loaded program and most people do really well!

🛑Stop telling patients with lateral hip pain to stretch! ✋
One of the leading causes of lateral hip pain is Gluteal Tendinopathy. This is an insertional tendinopathy and is aggravated by stretching movements that work into adduction, especially if combined with flexion or rotation.
The classic piriformis stretch or pigeon pose are often suggested for patients with symptoms in this region but actually tend to make things worse!
What if the patient likes to stretch or enjoys yoga? It’s not harmful but it’s best to reduce the intensity and duration of the stretch or focus on other areas, especially when symptoms are irritable.
As things improve people can gradually return to their goal activities (including stretches) and progress based on symptom response. This is important as we’re not suggesting long term avoidance just short term activity modification.
What do you like to recommend to patients with lateral hip pain instead of stretching? Let me know in the comments 🗣️

Our Story

Hi there!

I’m a Macquarie University (Sydney) graduate from 2012. I have an approach to MSK health that is patient centred as opposed to practitioner centred, which means I draw on a multitude of techniques and modalities that suit each individual. I strive to make this an overall empowering process and aim to be a true advocate for you - the patient. I'll work closely with you to achieve your health goals quickly and strongly encourage a team effort between myself, you, your GP and any other health care provider that may need to be involved including physiotherapists, podiatrists, exercise physiologists, massage therapists, sports trainers etc.... Plus, I like to take the time to get to know you, which helps me understand what is really going on.

I have a special interest in managing chronic pain. I like to delve into understanding the neurology and mechanics behind why injuries occur and try to relay this and educate my patients on what is going on in their body. This helps to reduce the fear of unknown, which goes a long way in helping you through difficult periods of pain and disability. I’m also a bit of a research nerd – as research gives us clinical certainty, therefore i'm dedicated to keeping up to date with the latest scientific and medical knowledge.

Outside of work I am a Yogi, an adventurer and a music lover.
