Location: Myrtleford Pharmacy
Time: 9am-6pm by APPOINTMENT ONLY
Eligibility by age: 12-59

Please call up to enquire and book appointments, Information and consent forms can be collected from the pharmacy and MUST BE FILLED PRIOR to coming in for your appointment.( NO EXCEPTIONS )
