Exercise is important in the management of osteoarthritis as it improves cartilage nutrition, increases strength, can improve movement patterns and help optimise loading of the joint. But how does exercise actually help?

Your cartilage receives nutrition from the surrounding synovial fluid. Loading and exercise helps boost nutrition and cartilage regeneration. Picture a sponge (cartilage) sitting in a bucket (joint capsule) of water (synovial fluid). When you squeeze the sponge (apply load to your cartilage) waste products are pushed out. When you release the sponge (remove load from cartilage) the water (nutrients) are sucked back in. This turnover provides nutrition and promotes regeneration of the cartilage.

Love your daily walks but don’t love the soreness afterwards?

If you have osteoarthritis, you can get back into walking again. The key is building a strong foundation through neuromuscular exercise, pacing and correctly managing loads. GLA:D Australia research shows that after completing the 6 week GLA:D program participants pain intensity had decreased 27-33% and their walking speed had increased by 12-14%.

Everyone wants to do more of the things that they love. Manage your osteoarthritis and get back to the things that bring joy to your life.

GLA:D was developed in Denmark in 2013 by Professor Ewa Roos and Professor Soren Skou. GLA:D stands for Good Living with osteoArthritis Denmark and was designed to provide high quality care by bridging the gap between osteoarthritis treatment guidelines and clinical practice.

GLA:D is an effective treatment for both hip and knee osteoarthritis. After completing a 6 week GLA:D program
- Average pain intensity decreased (33% for knee and 27% for hip arthritis)
- Reduced use of pain killers (49% of knee and 45% of hip participants reports they are using ‘less’ or ‘much less’ medication such as paracetamol, NSAIDS, opioids)
- Improved physical function (27% increase in timed STS test and 12-14% increase in walking speed)
- Higher quality of life (29% increase for knee and 20% increase for hip OA participants)

Research has found that a mild increase in pain during and following exercise is not harmful to cartilage. This pain should be at an ‘acceptable’ level and should settle within 24 hours (however this can take a little longer at the beginning). Studies have found that exercise is safe and effective even in severe osteoarthritis.

Correctly managing loads and increasing training gradually is essential for success when introducing a new activity and/or resuming an old hobby. Current best practice guidelines, both nationally and internationally, recommend exercise, education and weight management for treating osteoarthritis.
