Hunter Primary Care is a not-for-profit organisation located in Newcastle NSW. For over 30 years we have been supporting people to live a healthy life by delivering a range of health and wellbeing services to the Newcastle and Hunter community. Our services include -
• Aboriginal & Torres Strait Islander Health Services including care coordination and chronic disease management, mental health care coordination and psychology services
• Aged Care services including Aged Care Emergency (ACE), Aged Care Nurse Practitioner, Aged Care Clinical Placements Program and psychology services
• Allied Health services including Priority Allied Health, Multidisciplinary Lifestyle Community Groups and Coordinated Respiratory Support
• GP Access After Hours
• Mental Health Services including headspace Newcastle, Commonwealth Psychosocial Support and psychology services
• NDIS Services including Group Programs, Level 3 Specialist Support Coordination, Positive Behaviour Support, Psychology and Allied Health including dietitians, occupational therapists, exercise physiologists and social workers. Connect.

The Grounded Path is a new NDIS group program that helps people learn simple ways to manage stress, build routines and support their emotional wellbeing. The program includes mindfulness activities, time outdoors and group discussions, led by accredited social workers.

Our General Manager of NDIS & Chronic Disease, Claudine, joined our partners at Yakuway Indigenous Corporation to attend a powerful community event at the Hunter Medical Research Institute (HMRI) Imaging Centre earlier this week. The Story of Nakiliko Malang - Seeing Together celebrated the launch of a world-leading MRI upgrade and the growing strength of Indigenous-led research and health collaboration.
The event reinforced what we know to be true - the most effective First Nations health outcomes are community-owned, culturally grounded and co-designed with Aboriginal peoples. This is the same model guiding the work Hunter Primary Care and Yakuway are building together: integrating wellbeing, culture, research and healthcare to create better outcomes for our community.

Meet Sam, Behaviour Support Practitioner and Accredited Social Worker who brings compassion, lived experience and evidence-based practice to her work. Sam supports people living with psychosocial disability using a trauma-informed, recovery-oriented and client-centred approach, drawing on ACT and DBT strategies tailored to each person and their supports.
Sam is also a published academic author and is deeply committed to continuous learning and quality improvement. Alongside her professional expertise, Sam brings valuable lived experience as a carer, allowing her to work with empathy, insight and respect.
